摘  要:目的 探讨激素所致股骨头缺血坏死的防治。方法 作者选用新西兰大白兔23只,随机分为3组,激素治疗组8只;激素加氯笨丁酯(clolibrate)治疗组8只;对照组7只。结果治疗后6周发现,激素治疗组血胆固醇和甘油三酯明显增加,加用氯苯丁酯组血胆固醇和甘油三酯增加明显受到抑制,尤以甘油三酯更为明显。激素治疗组出现肝细胞脂肪变性;股骨头骨细胞内出现脂滴,并使骨细胞核受压,核固缩,甚至核消失。激素加用氯苯丁酯组肝细胞基本正常;股骨头骨细胞亦基本正常,细胞内未发现脂滴。结论通过氯苯丁酯的降血脂作用可以改善激素所致的脂质代谢紊乱,从而抑制其对股骨头骨细胞的损害,防止激素所致股骨头缺血的坏死。Objective We investigated experimentally the prevention ofsteroid-induced osteonecrosis of the femoral head . Methods Twenty-three rabbits were divided into three groups: ( Group S ) steroid treated , ( Group T. ) steroid and clofibrate treated , and controls. Serological tests ( cholesterol and triglyceride ) , histology and histochemicai studies. and electron microacopic examination were performed six weeks after treatment. Results The serum cholestelol and TG levels increased obviously in Group S. Although Group T animals also showed hyperlipidemia, their serum cholesterol and TG levels were much lower than those for Group 5. Group 5 animals demonstrated fatty degeneration of the liver. Lipid droplets were seen in the osteocytes of the femoral heads of these animals , which compressed the nucleus of the osteocytes along with nuclear pyknosis or absent nuclei. However , the constructures of liver cells and osteocytes of the femoral head were almost normal in Group T. Lipid containing ostescytes were not seen in these anamals. Conclusion These observations demonstrated that clofibrate decreased hyper- lipidaemia , modified lipid metabolism , and diminished steroid-induced damage to the osteocytes. This finding may be a guide to protect against steroid-induced osteonecrosis of the femoral head .
