摘 要:目的观察急性脑出血后自由基反应并探讨依达拉奉对脑出血的治疗作用。方法检测65例急性脑出血患者和100例健康对照者血浆一氧化氮(P-NO)、维生素C(P-VC)、维生素E(P-VE)、Β-胡萝卜素(P-Β-CAR)和过氧化脂质(P-LPO)含量以及红细胞超氧化物歧化酶(E-SOD)、过氧化氢酶(E-CAT)、谷胱甘肽过氧化物酶(E-GSH-PX)活性和过氧化脂质(E-LPO)含量。并将前者随机分为常规治疗组和依达拉奉治疗组,观察并比较治疗前及治疗后第3天、第7天两组上述各指标的变化情况。结果与正常对照组比较,患者组P-NO、P-LPO、E-LPO均值显著升高(P<0.01),P-VC、P-VE、P-Β-CAR、E-SOD、E-CAT、E-GSH-PX均值显著降低(P<0.01);与常规治疗组相比,依达拉奉治疗组在第3天后P-NO、P-LPO、E-LPO均值显著下降(P<0.01),P-VC、P-VE、P-Β-CAR、E-SOD、E-CAT、E-GSH-PX均值显著升高(P<0.01),治疗7天后,差异减小,但部分指标仍有显著统计学差异。结论急性脑出血时,人体内自由基反应病理性加剧,而依达拉奉可以清除过剩自由基,对脑出血患者的氧化损伤具有保护作用。Objective To observe the free radical reaction following cerebral hemorrhage and to explore the protective effect of Edaravone during this course. Methods 65 patients with acute cerebral hemorrhage(patient group) and 100 age- and sex-matched healthy people(healthy control group) were enrolled in this study, The former was further randomly divided into two groups: routine treatment group and Edaravone group. Plasma levels of nitric oxide(P-NO), vitamin C(P-VC), vitamin E(P-VE),β-earotene(P-β-CAR) and lipoperoxide(P-LPO), activities of superoxide dismutase (E-SOD), catalase (E-CAT), glutathioneperoxidase (E-GSH-Px) and lipoperoxide level (E-LPO) in erythrocytes of healthy control group were determined by spectrophotometry. The level and activity changes of these items before treatment and the 3rd and 7th day after management were also recorded both in routine treatment group and edaravone group, Results Compared with healthy control group, the levels of P-NO, P-LPO and E-LPO in patient group significantly inereased(P〈0.01), while those of P-VC, P-VE,P-β-CAR,E-SOD, E-CAT and E-GStf-Px signifieantly decreased (P〈0.01) on 3 days after treatment, the levels of P-NO, P-LPO and E-LPO in Edaravone group were obviously lower than those in routine treatment group(P〈0.O1) while the average P-VC, P-VE,P-β-CAR,E-SOD, E-CAT and E-GSH-Px had significantly rised(P〈0.01), On 7 days after treatment, the differences between the two groups became lessened, Conclusion In patients with acute cerebral hemorrhage there are aggravated pathological oxidative reactions which can be effectively alleviated by Edaravone. It has been shown that has a protective effect on oxidative damage in acute cerebral hemorrhage.
