超声引导下经皮肺穿刺活检的临床应用探讨

摘　　要：目的探讨超声引导下经皮肺穿刺活检术的临床应用方法及价值。方法应用Aloka SSD 500超声诊断仪，配置专用穿刺架，选用18—20 G切割式自动活检穿刺针，在B超引导下对39例胸部影像学检查显示的肺周边型肿块需明确诊断者行超声引导下经皮肺穿刺活检术。结果39例病人均穿刺成功，成功率100％。病理诊断结果：肺癌28例（其中鳞癌14例，腺癌7例，小细胞癌4例，大细胞癌1例，难以分类的恶性肿瘤细胞2例），结核6例，炎症5例。术后随访，诊断准确率达97％。结论超声引导下经皮肺穿刺活检术具有定位准确、操作简便、安全性好、并发症少及重复性强等优点，可作为肺周边型肿块的首选诊断方法。Objective To research the clinical application and the value ofultrasound - guided pereutaneous biopsy of the lung tissues. Methods Thirty- nine patients with pulmonary local lesion were applied pereutaneous lung biopsy using AlokaSSD - 500 ultrasonic diagnusing instrument , the special puncturing set and 18 - 20 G automatic puncturing needle under ultrasound guiding. Results They are all successful in all of the 39 patients ,and the achievement ratio is 100%. In the 39 patients,28 were diagnosed as lung cancer（ including half were squamous cell carcinoma,7 with adenocareinoma,4 with small cell lung careinoma（SCLC）, 1 with large cell lung careinoma（LCLC）and two were unclassified）.The others were including of 6 tuberculosis cases, and 5 with inflammation .The diagnostic accuracy was 97 %. Conclusion Ultrasound - guided percutaneous transthoracic needle aspiration biopsy has the advantages of accuracy, convenience safety, well reproducible and without serious complications, and can be the first choice for diagnosis of the pulmonary local lesion.
