炎性及肿瘤转移淋巴结的超声造影实验研究

摘　　要：目的探讨正常、炎性和肿瘤转移淋巴结超声造影前后回声强度的变化和显像效果。方法15只健康新西兰大白兔随机分为3组:肿瘤淋巴结转移组、炎性淋巴结组和正常淋巴结组。模型成功后,各组分别经足掌垫皮下注入自制表面活性剂类超声微泡造影剂,并按摩足底,用谐波显像方式观察各组腘窝淋巴结造影效果。结果造影前3组的回声强度值之间均有显著差别(P<0.05),肿瘤转移组最低,炎性组次之,正常组最高;造影后各组淋巴结回声强度均有不同程度增强,肿瘤转移组的视觉增强效果较其余两组低,其回声强度值低于其余两组,并可见不规则的充盈缺损,炎性组和正常组的回声强度比较无差别。结论声像图回声强度可在造影前区分肿瘤转移、炎性和正常的淋巴结,微泡造影后能更清楚显示其差别。Objective To explore the enhancement effect of self-made fluorocabon-filled surfactant contrast agent on inflammatory and VX2 carcinoma metastatic lymph nodes of rabbits. Methods Fifteen healthy adult New-Zealand white rabbits were divided into 3 groups randomly,including groups of VX2 carcinoma metastatic lymph nodes,lymph nodes of staphylococcal infection and normal lymph nodes. 0.5 ml self-made contrast agent was administered into the foot pad of each rabbit 2 times. Second harmonic mode（5.2/10. 5 MHz） was performed. The morphology changes of lymph nodes which were stained by HE was observed under a light microscope. Results Quantitative analysis showed there were significant differences among three groups before administration of contrast material （ P 〈 0. 05）. The grey scale pattern was highest in normal lymph nodes, intermediate in inflamatory lymph nodes and lowest in metastatic lymph nodes. After 0. 5 ml of contrast material into right footpad, the optical degree of enhancement in metastatic lymph nodes was lower than that of the other groups. The grey scale pattern of metastatic lymph nodes was lowest when compared to that of the other groups. The anomalo- filling defects could be seen in some lymph nodes of metastatic group. Conclusions Echo intensity can be applied to distinguish the inflammatory, VX2 carcinoma metastatic and normal lymph nodes before administration. Ultrasonic contrast agent in coordination with echo in tensity are promising methods for demonstration and evaluation of inflammatory and metastatic lymph nodes together.
