摘  要:目的:观察给予外源性重组人红细胞生成素对新生大鼠缺氧缺血性脑损伤后神经细胞凋亡的影响。方法:实验于2005-03/06在解放军第四军医大学病理实验室进行。①取新生7dSD大鼠72只,随机分为3组(n=24),假手术组、模型组和促红细胞生成素组,各组又分为造模后6,24,48,72h组4个亚组,每组6只大鼠。②促红细胞生成素组和模型组大鼠结扎右颈动脉,手术后即刻分别腹腔内注射重组人红细胞生成素注射液(10U/g)及同体积生理盐水,手术后2h两组大鼠吸入体积分数为0.08氧气和体积分数为0.92氮气的混合气体2h,建立缺氧缺血脑损伤新生鼠模型及促红细胞生成素治疗模型。假手术组分离动脉不结扎,不缺氧。③各组大鼠在造模后相应时间点断头处死,采用TUNEL法检测大脑海马区神经细胞凋亡情况。结果:经补充后72只进入结果分析。①模型组6h右侧海马CA1区出现凋亡细胞[(19.10±5.90)个],24h显著增高,48h达高峰[(65.70±8.33)个]后逐渐下降,但72h仍高于假手术组[(28.70±5.74),(0.60±0.84)个,F=71.587,P<0.01]。②促红细胞生成素组各个时间点CA1区的凋亡细胞数均少于模型组(P<0.01),尤其在24h最明显[(28.20±7.77),(60.30±7.75)个,t=-9.251,P<0.01]。结论:外源性重组人红细胞生成素能抑制缺氧缺血性脑损伤后的神经细胞凋亡,对脑组织确有保护作用。AIM: To observe the effect of exogenous recombinant human erythropoietin (rhEPO) on apoptosis of neural cell after hypoxic-ischemic brain damage (HIBD) in neonatal rats. METHODS: The experiment was done in Research Section of Pathology of Fourth Military Medical University of Chinese PLA between March and June 2005. ①Seventy-two seven-day-old SD rats were selected and divided randomly into three groups(n=24): Sham operation group, model group and EPO group. Then each group was divided into four subsets: 6, 24, 48 and 72 hours groups with 6 rats in each group. ②Right carotid artery of rats in the EPO group and the model group was ligated. Parenteral solution of rhEPO (10 U/g) and saline at the same volume were injected into rats through intraperitoneal injection immediately after operation. Mixture of oxygen with volume fraction of 0.08 and nitrogen fume with volume fraction of 0.92 was sucked in rats of the two groups 2 hours after operation to establish neonatal rat models with hypoxic ischemic brain injury and models in the treatment of EPO. Artery was separated in the sham operation group, non-deligation but with enough oxygen. ③Rats were killed at corresponding time point after building models. Terminal deoxynucleotidyl transferase mediated dUTP-biotin nick end labeling (TUNEL) staining was used to detect the neurons apoptosis in hippocampus RESULTS: After supplement, 72 SD rats were involved in the result analysis.①In the model group, apoptosis cell of neurons in CA1 region of right hippocampus was observed at 6 hours [(19.10±5.90) piece] and increased obviously at 24 hours and reached the peak at 48 hours [(65.70 ±8.33) piece],and then decreased at 72 hours, which was still higher than that in sham operation group [(28.70±5.74), (0.60±0.84) piece,F=71.587, P 〈 0.01]. ②In the EPO group, the number of apoptotic neurons was fewer than that in the model group at every time point at CA1 region (P 〈 0.01), especially at 24 hours [(28.20±7.77�
