摘 要:超声消融术是正在兴起的一种新的冠心病导管介入治疗方法。本文对人离体动脉粥样硬化斑块的研究表明,超声能够有效地清除斑块,在常规剂量下,不会损伤正常血管壁。超声消融产生的碎片较小,不会造成远端血管栓塞。The purposes of this study were to measure the relation between ultrasonic disruption and thermal damage of arterial wall and to measure the size of debris particles of disruption. Five human atheroscle-rotic arteries were studied in vitro Both 25 atherosclerotic plaques and 25 normai tissues were disrupted with 24 ±1 kHz,30W,50W and 60W continuous-wave ultrasound.The plaque disruption time, perforation time of aortic wall with or without plaques(sec/cm2)and the debris particles of disruption were measured.The mean time of plaque disruption was 104.74±40.79 sec/cm2 nder 30W. The meanxize of the debris particles wa,7.5±2.8μ.The perforation times of aortic wall with plaques and normal aortic wall were studied by 30W,50W, 60W,For plaque,needed 312±11.4,147±10.8, 60±15.0 sec. respectively;for normal aortic wall,needed 635±12. 6,303±34.2, 174±8.2 sec respectively. Ultrasound could effectively ablate plaques with little damage to normal arterial wall under routine ultrasound power output.The arterial walls with plaques were more susceptible to ul-rasonic disruption than normal arterial wall. The debri,particles of disruption were too small to be resulted in distal embolism of arteries.
