经阴道二维，彩色血流多普勒在早期妊娠的研究

摘　　要：本文研究目的是了解经阴道二维，彩色血流多普勒在早期妊娠的诊断价值。１４９例人流患者自愿接受本项检查。研究结果表明，经阴道超声（ＴＶＳ）对早期妊娠结构，如孕囊、卵黄囊、胎心搏动，胎头及胎体辨别、颅骨、脊柱、心脏等均较经腹部超声（ＴＡＳ），提前１～２周显示，且结构更清晰。经阴道彩色多普勒（ＴＶＣＤ）在早期妊娠全部病例均可测到双侧子宫动脉血流，９７％测到滋养层血流，７８％可测到妊娠黄体血流，５周以后９２％可测到胎儿主动脉血流，７周以后８８％可测到脐动脉血流，９周以后可测到脑动脉血流。A study was undertaken to evaluate the ability of transvaginal 2-D and color Doppler in early pregnancy. The examinations were performed in 149 volunteer pregnant women whose menstrual age ranged from thirty days to 12 weeks.The results showed that consistant detections of respective structure were achieved at the following circumstances: The yolk sac,fetal pole and the cardiac pulsation at 4 weeks to 6 weeks; fetal sagittal contours and limbs pole at 7 weeks;the face palate and 4th ventricles of brain at 8 weeks to 10 weeks;shorter and thinner long bones, the feet and the toes at 10 weeks to 11 weeks; the four-chamber view of heart seen at 12 weeks.In early pregnancy blood flow, we were able to locate both uterine arteries in all cases,and continuous diastolic shift signal was found. Peritrophoblastic flow was observed with success rate of 97%. Cou pus luteum flow was found in 78%cases.Blood flow in fetal aorta was visualized by color Doppler Starting from the fifth week,and flow in the umbilical artery at the seventh week.Intracranial blood flow could be visualized starting from the ninth week.Distolic flow velocity waveform in these vessels was absent before 11 weeks.
