摘  要:目的观察非静脉转流下猪原位肝移植术监测指标的变化。方法 12对健康猪在全麻下,手术中不应用静脉转流,行原位肝脏移植。颈动脉、静脉穿刺,在不同时间段术中分别检测受体猪的心率(HR)、中心静脉压 (GVP)、平均动脉压(MAP),动脉血气分析和电解质K+、Na+、Ca2+的变化。结果受体猪无肝期和新肝期HR明显加快,MAP、CVP无肝期下降明显,动脉血pH值下降,碱剩余(BE)负值加大(P<0.05)。新肝期随肝循环的开放,酸中毒进一步加重,血钾明显升高(P<0.05)。结论非转流下猪肝脏移植术随手术过程的进展,HR加快、 MAP、CVP下降,pH值下降、血钾升高、酸中毒加重,应及时防治。Objective: To investgate hemodynamic and biochemistric changes during orthotopic liver transplantation in pigs with no-bypass. Methods: Twelve pairs of healthy pigs were anesthetized and intubated, and no-bypass was done during orthotopic liver transplantation. CVPs and MAPs were monitored bycannulating artery or vein, The arterial blood was sampled for monitoring arterial gas .tensions, Na^+ , K^+ and Ca^2+ . Results: During anhepatic and neohepatic stage, HR increased distinctly. CVP and MAP decreased at anhepatie stage. And pH decreased, base deficit increased significantly (P 〈0.05). During neohepatic stage with blood reperfusin, acidosis became more sever, K + increased significantly ( P 〈0.05). Conclusion: During operation of liver transplantation, HR and K+ increse, MAP and CVP and pH decrease. Then metabolic acidosis develops progressively and they should be treated in time.
