腓肠神经营养血管皮瓣加闭式灌注引流修复足踝部感染创面的护理

摘　　要：自的：讨论足踝部软组织缺损感染创面术后的护理。方法：回顾性总结腓肠神经营养血管皮瓣修复18例足踝部软组织缺损感染创面，术后持续以生理盐水加庆大霉素液灌注引流8～20d，行个性化护理，观察皮瓣血运。结果：18例皮瓣全部存活．术后随访2～20个月，感染无复发，皮瓣色泽、质地弹性良好，但感觉差。结论：腓肠神经营养血管逆行岛状皮瓣加闭式灌注冲洗引流法术后护理操作简便，可Ⅰ期修复足踝部软组织缺损并感染的创面。Objective：To study the nursing measures of the soft-tissue defects and infective wound of ankle and foot after operation. Methods：It was analyzed retrospectively that the distally based superficial reversed sural neurovaseularflap were designed and dissected on the posterior aspect of the leg,and 18 patients were treated with this flap,who had soft-tissue defects and infective wound of ankle and foot. After the operation,continuous perfusion drainage with physiological saline and gentamicin lasted 8～20 d ,and individualized nursing were also used by expression of sympathy, and the flaps＇blood circulation was observed. Results ： All flaps survived. Through 2 to 20 months＇postoperative followup ,it is found that the infection was not recurrence ,and the color ,texture and appearance of the flaps were good, but sensory function of the flaps were poor. Conclusion：The reversed sural neurovascular flap and perfusion drainage are convenient in nursing after operation. The wound can heal up as the patients treated with this flap and perfusion drainage and effective postoperative nursing cooperation.
