中心型肥胖患者内皮舒张功能与血清瘦素相关

摘　　要：目的探讨中心型肥胖患者内皮依赖性舒张功能异常与血清瘦素的关系。方法按配对方法分为中心型肥胖组和对照组,用放射免疫法检测血清瘦素水平,用高分辨超声技术检测肱动脉内皮舒张功能。结果中心型肥胖组肱动脉内皮依赖性舒张功能较对照组明显减退(6.71%±3.60%比13.81%±3.71%,P<0.01),非内皮依赖性舒张功能差异无显著性(19.71%±6.63%比18.60%±6.35%,P>0.05)。中心型肥胖组血清瘦素水平明显高于对照组,男性肥胖组与男性对照组比较(8.63±3.73μg/L比3.05±1.56μg/L,P<0.01)、女性肥胖组与女性对照组比较(16.73±6.93μg/L比7.93±3.66μg/L,P<0.01)差异均有显著性。Pearson相关分析显示肱动脉内皮依赖性舒张功能与血清瘦素水平呈显著负相关。结论中心型肥胖者存在内皮依赖性舒张功能异常,它与血清瘦素水平的增高有关。Aim To study the relationship between endotbelium-dependent vasodilatation in abdominally obese patients and serum leptin. Methods The patients were matched into abdominally obese group and controll group according to age and sex. The concentration of leptin was assayed by radioimmunoassay. The endotbelium-depondent vasedilatation was measured by hlgh-resolution ultrasound in the two groups. Results The brachial flow-mediated vasodilatation in abdominally obese group was much lower compared with that in control group （6.71%±3.60% vs13.81%±3.71%, P〈0.01）. However, there was no significant difference in response to nitroglycerin between the two groups （ 19.71% ± 6.63% vs 18.60% ±6.35%, P 〉 0.05）. The concentration of serum leptin in abdominally obese group was much higher than that in control group, the obese male vs the controlled male is 8.63 ± 3.73 μg/L vs 3.05 ± 1.56μg/L （ P 〈 0.01） ; the obese female vs the controlled female is 16.73 ± 6.93 μg/L vs 7.93 ± 3.66 μg/L （ P 〈 0.01 }. The brachial flow-mediated vasodilatation was negatively correlated with the concentration of serum leptin according to Pearson correlation analysis. Conclusions The endotbelium-dependent vasedilatation dysfunction in abdominally obese patients is negatively correlated with the increase of the concentration of serum leptin.
