摘 要:目的:研究胎儿三维超声在临床中的应用,并与二维超声进行比较。材料和方法:孕妇220例,孕龄6~12周75例,13-28周105例,29~40周40例。使用的仪器为三维超声工作站和三维彩超机容积探头。图像采集方法:用扇形自由扫查方式匀速采集图像;或用容积探头自动采集闭像。图像采集后,根据研究的需要进行表面或透明三维重建。结果:220例中有205例三维图像显示满意,成功率93.2%;查出无脑儿,脑积水、内脏外翻、死胎各1例,胚胎停育5例,与二维图像比较,二维图像更能清楚地显示胚胎发育过程中三维形态的演变,能清楚地显示胎儿头面体表结构及脊柱肋骨结构,图像直观,立体感强。结论:胎儿三维超声明显优于二维超声,能直现显示胎儿头面体农结构及先天畸形,具有极高的临床应用价值。Objective: Study or Application value of three-dimensional ultrasound ( 3D US ) of the fetal in the clinic. The images of twodimensional ultrasound ( 2D US ) were compared with ones of 3D US. Materials and Methods: In this study, 220 pregnancies were observed including 75 cases with 6-12 weeks gestational age, 105 cases with 13-28 weeks, and 411 cases with 29-411 weeks. Applying devices were 311 US workstation connected with ordinary 2D US device anti 3D US device with special volume probe . These fetals were first examined with ordinary 2D US, then were examined with 3D US selected best US window, Imaging data of 3D US were acquired throug free-hang scanning with fan scanning methods. Surface rendering and transparent rendering were used according to studing purpose, Results: 3D US images were obtained successfully of 2115 cases in 220 cases ( 93.2%). Nine fetal anomalies were detected including anencephalus, hydrocephalus, eversion of veserer and fetal dealh 1 case respectively, embryo developmental arrest 5 cases. 3D US imaging were superior to 2D imaging evidendy. The shape of embryo, surface structure fetal face and body, and spine and rib were showed clearly. Conclusion: 3D US of fetal can provide more plentiful information in display of surface structure fetal face and body and congenital anornaties than 2D US. It plays an important part in the clinical applications.
