甲亢中散发型无痛性甲状腺炎的临床研究

摘　　要：目的探讨甲状腺功能亢进(甲亢)症中的散发型无痛性甲状腺炎(STP)的发病率及实验室检查特征。方法从以典型甲亢症状初诊的168例患者中,鉴别出28例STP并与Graves病及桥本氏病相比较。结果STP的发病率为16.7%,其相关的实验室检查不同于Graves病及桥本氏病。SPT组TT3、TT4、FT3、FT4、TSH与Graves病组及桥本氏病组相比较无显著性差异(P>0.05)。SPT组TGAb、TMAb与Graves病组相比较有显著性差异(P<0.05),与桥本氏病组相比较差异更明显(P<0.01)。SPT组TRAb与Graves病组相比较差异非常明显(P<0.01),与桥本氏病组相比较无显著性差异(P>0.05)。SPT组血沉(ESR)与Graves病组及桥本氏病组相比较无显著性差异(P>0.05)。SPT组的甲状腺摄131I率(RAIU)均低于正常与Graves病组相比较有非常明显的差异(P<0.01),与桥本氏病组相比较差异也较明显(P<0.05)。结论甲亢中的STP仅次于Graves病而较桥本氏病多发,实验室检查尤其是RAIU检测对诊断具有鉴别意义。Objective To investigate the incidence of sporadic type of painless thyroiditis（SPT） and to analyse its laboratory characteristics. Methods Twenty-eight patients with SPT,who were selected from 168 patients with typical thyrotoxicosis, were compared against patients with Graves disease and Hoshimotos disease. Results The incidence of SPT was 16.7%,the laboratory examination results in SPT differed from those of Graves disease and Hoshimotos disease. In the SPT group, no conspicuous distinction was found in TT3 ,TT4 ,FT3, FT4 ,TSH against that of Graves disease and Hoshimotos disease（P〉0.05）; TGAb and TMAb were obviously different from that of Graves disease（P〈0.05）and Hoshimotos disease（P〈0.01） ;TRAB resembled but differed from that of Graves disease（P〈0.01）and that of Hoshimotos disease（P〉0.05）;ESR was close to that of Graves disease and Hoshimotos disease（P〉0.05） ;RAIU was lower than normal and distinctly different from that of Graves disease（P〈0.01）and Hoshimotos disease（P〈0.05）. Conclusion The incidence of SPT ranks only after Graves disease but before Hoshimotos disease among patients with hyperthyroidism;laboratory examinations, especially of RAIU, have significant implications for diagnosis.
