摘  要:目的评估肾通注射液对家兔急性肾衰竭(ARF)的防治作用,并初步探讨其机制。方法将36只健康雄性大耳白兔随机分为正常组、模型组和治疗组,每组12只。禁水12h后,模型组和治疗组动物双后下肢肌注50%甘油等渗盐水(15ml/kg)以复制急性肾衰,正常组同法注射等容量生理盐水。造模1h后,治疗组腹腔注射肾通注射液2ml/kg,8h后重复给药1次;正常组和模型组同时腹腔注射等容量5%葡萄糖。通过生化法监测肾功能,彩色多普勒超声动态观察各组家兔第24、48、72h肾血流动力学变化,以放免法测定血浆和肾局部内皮素(ET-1)含量。结果治疗组肾血流量较模型组显著升高,各级动脉阻力指数显著降低,肾功能各项指标均有显著改善,肾血管活性物质产生明显变化,血ET显著下降,肾局部ET与模型组比较差异更为显著。结论肾通注射液可增加实验性ARF家兔的肾脏血液供应,提高肾小球滤过率,保护肾功能,而调节ET-1水平是其重要的作用机制之一。Objective To investigate the effects of Shentong injection on renal hemodynamics in rabbits with acute renal failure(ARF) and to explore its mechanisms. Methods Thirty six male rabbits were randomly divided into three groups, 12 rabbits in each: normal group(N), model group ( M) and treated group(T). In group M and T, 50M glycerin physiological saline(15ml/kg)were injected in lower exetremities. 24, 48 and 72h after glycerin-induced ARF were made, renal hemodynamics were measured by color Doppler ultrosound, renal functional parameters were monitored by biochemical method, plasma and renal local ET-1 were investigated respectively. Results Compare with normal group, renal blood flow(RBF) and creatinine clearence rate(Ccr) were remarkably reduced in model group (P〈0. 01), plasma and renal local ET-1 were increased, the differences were significant (P〈0. 01). The concentration of renal local ET-1 changes more obvious than that of plasma. In Shentong treated group, RBF and Ccr were significantly higher than that in group M, but renal local ET-1 decreased evidently. Conclusion Shentong injection can relax renal blood-vessels, relief the resistence of microcirculation, improve RBF and Ccr,and protect renal function. Regulating the level of ET-1, especially the renal local ET-1 ,may be one of important mechanisms.
