婴幼儿腹腔镜手术麻醉方法的选择

摘  要:目的探讨非气管插管静脉全麻用于婴幼儿腹腔镜疝内环结扎术的可行性、安全性、实用性。方法60例行腹腔镜疝内环结扎术患儿,随机分为全麻插管组(Ⅰ组30例)和非插管组(Ⅱ组30例),连续监测HR、SPO2、PETCO2。结果两组气腹时HR均增快,但无统计学意义,放气后逐渐恢复至气腹前水平,Ⅱ组气腹后10min PETCO2明显高于麻醉前(P〈0.05)但尚在正常范围,组间比较亦无明显差异(P〉0.05);Ⅱ组气腹后的PETCO:高于明显Ⅰ组,差异显著(P〈0.05)。结论非气管插管全麻在婴幼儿腹腔镜疝内环结扎术中是一种比较理想的麻醉方法之一。Objective To study the feasibility , safety and practicability of combined intravenous anesthesia with exemption of the endotracheal intubation in laparoscopic circumferential wiring of infant indirect inguinal hernia. Methods 60 children with indirect inguinal hernia underwent circumferential wiring of the internal ring were randomly divided into group Ⅰ (30 cases received intravenons anesthesia with endotracheal intubation) and group Ⅱ (30 cases received the same anesthesia with exemption of the endotracheal intubation) . The HR , SPO2 and PETCO2 were recorded continously in each patient. Results The HR of both groups increased significantly after the anesthesia , and it returned to the preoperative level gradually after emptying of the intraperitoneal air. There was no statistical significance in HR between the two groups. 10 minutes after the establishing of pneumoperitoneum , the PETCO2 value increased significantly (P 〈 0.05) in group Ⅱ but still within the normal range. There was no stastistical difference in PETCO2 value between group Ⅰ and Ⅱ. The PETCO2 increased 10 minutes after the establishing of the pnemoperitoneum in group Ⅱ than that in group Ⅰ (P 〉 0.05 ). Conclusion intravenous anesthesia with exemption of the endotracheal intubation is leasable for laparoscopic circumferential internal ring wiring of infant indirect inguinal hernia.
