整合素α4β7的表达对重症胰腺炎大鼠肠道免疫功能的调节作用

摘　　要：目的探讨淋巴细胞归巢受体整合素α4β7的表达与重症急性胰腺炎(severeacutepancreatitis,SAP)大鼠肠道黏膜免疫功能的关系,分析SAP大鼠肠道黏膜免疫功能改变的可能机制。方法制备大鼠SAP模型,随机分为胰腺炎组和假手术组。分别于造模后24、48和72h取标本检测:鲎试剂法检测大鼠门静脉血内毒素水平、免疫组化方法检测并计数小肠黏膜固有层内CD3、CD4、CD8阳性T淋巴细胞百分数,流式细胞仪检测外周血中淋巴细胞归巢受体α4β7阳性淋巴细胞数的变化。结果SAP组大鼠各时段门静脉血内毒素水平显著升高,回肠末段黏膜固有层CD3+、CD4+T淋巴细胞百分数明显减少,CD4+/CD8+T淋巴细胞比值降低;SAP组24、48和72h大鼠外周血中α4β7阳性淋巴细胞数量显著降低。结论SAP大鼠早期即发生肠黏膜免疫功能明显下降,可能与SAP时大鼠外周血中α4β7阳性淋巴细胞数所占比例降低相关。[Objective] Discuss relationship between the expression of integrin α4β7-lymphocyte homing receptor and the intestinal mucosal immune function in experimental severe acute pancreatitis （SAP） rat. Analyze the possible mechanis,us of changes of intestinal immune function. [Methods] Male adult Wistar rats were randomly divided into pancreatitis group and sham-operation group. Animals were sacrificed at 24, 48 and 72 hours after models were made and biopsies were harvested; endotoxin concentration in portal vein was determined by limulus methods;CD3^＋, CD4^＋, CD8^＋T lymphocytes in intestinal mucosal lamina propria were examined by immunohistochemistry;integrin α4β7（＋） peripheral blood lymphocytes were determincd by flow cytometry. [Results] Compared to the control group, endotoxin concentration of plasma in portal vein rises to a higher level, percentage of CD3^＋, CD4^＋ Tlymphocytes subsets in the end of intestinal mucosal lamina propria redt,ced significantly, CD4^＋/CD8^＋ ratio decreased, integrin α4β7 （＋） lymphocyte in pcripheral blood was significantly reduced at 24, 48 and 72 hours in SAP grottp. [Conclusions] Intestinal inmmne sappression occurred in the early stage and may be the main reason for bacteria and endotoxin translocation in SAP rat. The possible mechanism maybe that there were less integrin α4β7（＋）T-lymphocyte in peripheral blood during SAP.
