不同剂量阿托法他汀早期治疗急性冠脉综合征对炎症因子及血脂的观察

摘　　要：目的观察急性冠脉综合征(ACS)患者使用两种剂量阿托法他汀(10和40mg)治疗1周后血浆C-反应蛋白(CRP)、胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)的变化。方法对住院确诊为ACS的62例患者按随机原则分别入选,常规治疗组(20例),不接受任何调脂治疗。阿托法他汀治疗组:分别接受10m(g21例)和40m(g21例)阿托法他汀治疗7d。并测定各组治疗前后CRP、TC、TG、LDL-C和HDL-C的变化。结果治疗7d后,10mg阿托法他汀组CRP、TC、TG和LDL-C均有所下降。但与常规治疗组比较差异无统计学意义。而40mg阿托法他汀治疗组则明显优于常规组,两组间CRP的下降幅度差异有统计学意义,但3组治疗前后各血脂成分的变化差异均无统计学意义。结论ACS患者大剂量阿托法他汀治疗7d后,可明显控制血浆炎症因子CRP,从而有利益于改善内皮功能,稳定动脉粥样斑块。[Objective] To observe whether intervention with two different dosage of atorvastain （10 mg and 40 mg） may benefit to plasma levels C-reactive protein （CRP）, the total plasma cholesterol（TC）, triglyeeride（TG）, low density lipoprotein （LDL-C）, high density lipoprotein in patients of acute coronary syndrome at seven days after treatment. [Methods] Patients with ACS were randomly assigned into three groups： the control group （n =20） treated without lipid-lowering drugs, 10 mg （n =21） and 40 mg （n =21） atorvastain group with seven days of treatment. Before and after the treatment, the plasma levels of CRP TC TG LDL-C and HDL-C were detected. [Results] The treatments of 10 mg atorvastain did not decrease significantly the levels of CRP, TC, TG, and LDL-C. Compared to the control group, levels of CRP were markedly reduced by 40 mg atorvastain （CRP 60.3% vs 5.8%）. No changes of lipid levels were observed in any group brfore and after treatment. [Conclusion] Treatments with hlgh-dose atorvastain （40 mg） for seven days could reduce the levels of inflammatory factor CRP, and in,prove the function of vascular endothelium ,which may promote the stabilization of atheroselerotic piaque.
