音乐放松想像训练对腹腔镜肝切除病人应激反应的影响

摘　　要：[目的]探讨音乐放松想像训练(MRIT)对腹腔镜肝切除病人手术应激的影响。[方法]将40例腹腔镜肝切除病人分为实验组和对照组,实验组在围手术期常规护理的基础上接受音乐放松想像训练,对照组则只接受常规护理。观察两组手术前后血压、心率、血清皮质醇、C-反应蛋白(CRP)及免疫球蛋白的变化。[结果]两组收缩压、血清CRP和皮质醇分别在术后第1天和第2天达峰值,但对照组较实验组升高更明显(P<0.05);两组舒张压和心率均无差异(P>0.05);两组血清IgA、IgG、IgM较术前降低,但组间无显著差异(P>0.05)。[结论]音乐放松想像训练用于腹腔镜肝切除病人围术期护理中,可减轻手术应激反应强度,但对应激引起的免疫抑制无明显影响。Objective： To probe into the influence of music relaxation and imagery training on operation stress of patients underwent hepatectomy via laparoscope. Method： A total of 40 patients after underwent laparoseopic hepatectomy were divided into both test and control group. Patients of test group were accepted music relaxation imagery exercises base on the perioperative routine nursing care. While patients of control group were only given routine care. Then changes of blood pressure, heart rate, serum hydrocortisone, CRP and immunoglobulins of all cases both before and after operation were observed. Result： Systolic pressure, cortisone level and CRP of both group cases were up to peak values on the first and the second postoperative day respectively. The above mentioned three indexes of control group cases were obviously higher than that of the test group （P〈 0.05）. However, there was no significant difference in terms diastolic pressure and heart rate between the two groups （PD0.05）. In terms of the serum IgA, IgG and IgM, postoperative patients of both groups were lower than that of before surgery. However there was no significant difference between the two groups patients （ P〉 0.05 ）. Conclusion： It suggested that to apply music relaxation and imagery training as perioperative nursing care for patients undergoing hepatetomy via laparoscope could relieve their operative stress reaction. And yet which has no obvious influence on stress- induced immune suppression among them.
