滋肾活血凉斑方加火把花根片对活动期系统性红斑狼疮患者生化指标的影响

摘　　要：目的:观察具有滋补肾阴、凉血活血、清热解毒、通络退斑功效的滋肾活血凉斑方加具有祛风除湿、舒筋活络、清热解毒功效的火把花根片对活动期系统性红斑狼疮患者疾病活动情况、血液流变学指标及阴虚热毒血瘀证候的影响。方法:①选择1998-01/2005-06重庆医科大学附属一院、重庆医科大学中医药学院附院中西医结合科诊治的中医辨证为“肾阴虚兼热毒血瘀”证的活动期系统性红斑狼疮门诊患者60例。男3例,女57例。患者均知情同意。患者被随机分为2组:治疗组(40例,男2例,女38例)和对照组(20例,男1例,女19例)。②治疗组:予滋肾活血凉斑方加火把花根片治疗(滋肾活血凉斑方基本组成:女贞子30g,旱莲草30g,知母15g,蚕砂15g,千里光30g,白花蛇舌草30g,银花藤30g,丹参30g,益母草15g,僵蚕15g等;火把花根片由重庆市中药研究院制药厂生产,国药准字Z20027411,0.18g/片)。滋肾活血凉斑方水煎服,1剂/d,分3次服用;火把花根片0.9g,3次/d,2个月后逐渐减量。共干预6个月。对照组:采用强的松标准疗程治疗。③治疗前及治疗6个月后分别对各组进行疾病活动评分(抽搐8分、精神异常8分、脑器质性症状8分、视力下降8分、颅神经受累8分、狼疮头痛8分、脑血管意外8分、血管炎8分、关节炎4分、肌炎4分、管型尿4分、血尿4分、蛋白尿4分、脓尿4分、新出皮疹2分、脱发2分、口腔溃疡2分、发热1分、血小板减少1分、白细胞减少1分。计总分)。治疗前和治疗6个月后检测疾病活动指标抗核抗体、抗双链DNA抗体、补体C3、血沉、尿常规、24h尿蛋白定量、肾功;采用全自动血液流变分析系统检测血液流变学指标。治疗6个月后进行阴虚热毒血瘀证疗效评估(显效:症状、体征明显改善,证候积分减少≥70%;有效:症状、体征均有好转,证候积分减少≥30%;无效:症状、体征无改善,证候积分减少<30%,甚或�AIM： To study the effects of zishen haoxue liangban decoction which can nourish kidney-yin, cool blood and promote blood circulation, clear away heat and detoxify, activate the channel and clear away speckle combined with huoba huagen which can expel wind and remove dampness evil, relieve rigidity of muscles and activate collaterals, clear away beat and detoxify on the disease activities, hemorheology and yinxu redu xueyu syndrome of systemic lupus erythematosus at active phase. METHODS： ① Sixty outpatients （3 males and 57 females） with yinxu redu xueyu syndrome systemic lupus erythematosus at active phase were selected from the Department of Integrated Chinese and Western Medicine, First Affiliated Hospital and the Affiliated Hospital of College of Traditional Chinese Medicine, Chongqing University of Medical Sciences from January 1998 to June 2005. All these subjects were randomly divided into the therapy group （n=40, 2 males and 38 females） and the control group （n=20, 1 males and 19 females）. ② Therapy group： Cases in therapy group were treated with zishen huoxue liangban decoction （containing： fructus ligustri lucidi 30 g, yerbadetajo herb 30 g, rhizoma anemarrhenae 15 g, excrementum bombycis 15 g, climbing groundsel herb 30 g, spreading hedyotis herb 30 g, japanese honeysuckle stem 30 g, red sage root 30 g, motherwort herb 15 g, bombyx batryticatus 15 g, etc.） and huoba huagen tablet （supported by the factory of Chongqing Academy of Chinese Material Medicine, Z20027411, 0.18 g/tablet）. Zishen huoxae liangban decoction： 1 potion per day, 3 times a day; hu, oba huagen tablet： 0.9 g, 3 times a day for 2 months, then diminished the dosage gradually to the 6^th months. Control group： Cases in control group were treated with standard period of prednisone. ③ To score for the systemic lupus erythematosus disease activity index betfore and 6 months after treatment： seizure 8, psychosis 8, organic brain syndrome 8, visual disturbance 8, cranial nervedisorder 8, lupus headache
