正常成人超声心动图的左室质量测值及其影响因素研究

摘　　要：目的确定正常成年人超声心动图对左室质量（left ventricular mass，LVM）的正常值，了解其影响因素。方法随机、整群、分层抽样调查广西南宁市685例18～85岁的正常成人，应用超声心动图测量LVM，分析其影响因素。结果不同影响因素的LVM测值结果见正文。LVM在文化程度、收入分层级别中无差异。但LVM在不同年龄、工作强度、收缩压、舒张压、体表面积分层中有显著性差异，P值分别〈0．01、0．05、0．001、0．001、0．001。测值与左室每搏量（SV）、体表面积呈正相关（r=0．4234、0．5833）；与左室射血分数（EF）呈负相关，r=-0．304。逐步回归分析提示SV、EF、体表面积、左室短轴缩短率（FS）、年龄、吸烟史、右房横径是LVM的影响因素。结论LVM正常值受多种因素的影响，应根据相关因素进行细化、分层。Objective To establish the left ventricular mass （LVM） normal ultrasonic value of heart by echocardiogram and approaching the affective factors. Methods A random, stratified cluster sampling overall survey was done in Nanning, 685 healthy adults were enrolled into this study. We measured LVM on echocardiogram and analyzed the affective factors. Results The results of LVM in different affective factors can be seen in the text. There was no significant difference between different degree of earning and education. there was significant differenc between different degree of age.job intension, systolic pressure ,diastolic pressure, body surface area in LVM （P〈0. 01, 0. 05, 0. 001, 0. 001, 0. 001, respectively We found LVM was positive relatively to SV and body surface area （r=0. 423 4, 0. 583 3 respectively）, negative relatively to EF, r= -0. 304. Multiple stepwise regression analysis showed that the SV, EF, body surface area, FS, age, history of smoking, RAD were independent parameters to LVM. Conclusions The normal value of left ventricular mass was affected by multiple factors, and it should be analysed attentively according to multiple factors.
