摘  要:目的:探索多层螺旋CT血管成像3D-MSCTA的技术及临床应用。方法:本文收集MSCTA血管成像44例,其中脑动脉血管MSCTA成像11例,肺动脉血管MSCTA成像21例,主动脉血管MSCTA成像6例,肝动脉血管MSCTA成像5例,髂动脉血管MSCTA成像1例。使用非离子型对比剂碘海醇100ml,对比剂注射速率为3~3.5ml/s。扫描延迟时间按照经验值,脑动脉15~20s,肺动脉10~15s,主动脉、肝动脉、及髂动脉等20~25s。扫描后原始图像数据送工作站做容积重建(VR)、最大密度投影(NIP)、多平面重组(MPR)、部份病例做曲面重组(CPR)等后处理。结果:11例脑血管MSCTA正常6例,肺动脉MSCTA(CTPA)21例,正常15例,5例肝动脉MSCTA,正常2例;脑血管动脉瘤4例,脑动静脉畸形(AVM)1例;肺动脉栓塞6例;胸主动脉夹层3例:升主动脉瘤1例;腹主动脉瘤2例;肝动脉瘤1例,肝癌1例,肝血管瘤1例;1例右髂总动脉瘤。结论:多层螺旋CT3D血管成像(3D-MSCTA)可清晰显示各部位的三维血管形态,可以多方位、多角度地旋转图像,全面立体直观地显示病灶的整体形态,对指导临床手术有很大的帮助。Objective:To study the technique of multi-slice spiral CT angiography and its clinical application. Methods:44 cases of multi-slice spiral CT angiography were analyzed. Among them 11 were of cerebral vascular, 21 of pulmonary vascular, 6 of aortic vascular, 5 of hepatic vascular and 1 of common iliac vascular. 100ml nonionic contrast medium (liohexol) was used. The rate of injection was 3 - 3.5ml/s. The applied empirical value of scan delay time was 15 - 20s for cerebral artery, 10 - 15s for pulmonary artery and 20 - 25s for aorta, hepatic artery and common iliac artery. The original scanned data were delivered to working-station for VR, MIP, MPR and CPR.Results:6 of the 11 cerebral vascular MSCTA, 15 of the 21 pulmonary vascular MSCTA and 2 of the 5 hepatic Vascular MSCTA were normal. The others were 4 intracranial aneurysm, 1 AVM, 6 pulmonary embolism, 1 hepatic aneurysm, 1 liver cancer, 1 hepatic hemangioma and 1 right common iliac artery aneurysm. In the 6 cases of aortic vascular MSCTA, 3 were thoracic aortic dissection, 1 ascending aorta aneurysm and 2 abdominal aortic aneurysm.Conclusion:Multi-slice spiral CT angiography (MSCTA) clearly shows all 3D vascular structures in the body. Revolving the photo in multi-orientation and multi-angle to completely display the three dimensional and visual focal shape. Therefore, MSCTA can he of great helpful to clinical operation.
