青年期阻塞性睡眠呼吸暂停低通气综合征患者的睡眠监测分析

摘　　要：目的:分析青年阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea-hypopnea syn-drome,OSAHS)患者的多导睡眠监测特点。方法:青年组50例、平均年龄(31.2±4.1)岁,中老年组199例、平均年龄(47.1±7.5)岁,进行两组间多导睡眠监测(polysomnography,PSG)资料的比较。结果:50例青年患者体块指数(body mass index,BMI)平均(30.0±4.3)kg/㎡。青年组呼吸暂停低通气指数(apnea hypop-nea index,AHI)平均(61.2±32.2)次/h明显高于中老年组患者(44.6±26.5)次/h(P<0.01)。非快速动眼睡眠期(non-rapid eye movement,NREM)所占百分比的比较显示:青年组Ⅰ期(53.9±18.3)%低于中老年组(61.5±18.9)%(P<0.05);青年组Ⅱ期(33.4±15.3)%、Ⅲ+Ⅳ期(3.0±6.9)%均高于中老年组Ⅱ期(23.3±17.0)%,Ⅲ+Ⅳ期(1.1±2.8)%(P<0.05)。快速动眼睡眠期(rapid eye movement,REM)和微觉醒指数在两组间差异无统计学意义。青年组<90%、<80%、<70%血氧降低次数明显高于中老年组(P<0.05),而最低血氧饱和度(lowest SaO2,LSaO2)和最长呼吸暂停时间在两组间差异无统计学意义。结论:就诊的青年OS-AHS患者呼吸紊乱及低血氧程度明显重于中老年患者,但睡眠结构紊乱的程度基本接近。Objective： To analyze the characteristics of polysomnography in youth with obstructive sleep apnea-hypopnea syndrome. Methods： 32-guide polysomnography was monitored in 50 youth and 199 middle-aged patients with obstructive sleep apnea-hypopnea syndrome, and the results were analyzed by polysmith. Results： The BMI for youth was （30.0±4.3） kg/m^2and（29.0±3.6） kg/m^3for middle-aged patients. AHI of youth was higher than that of middle-aged patients [（61.2 ±32.2）/h vs （44.6±26.5）/h] （P〈0.01）.The mean value of NREM stage ] in youth was lower than that of middle - aged patients [（53.9±18.3）% vs （61.5± 18,9）%1： of stage Ⅱand stage Ⅲ ± Ⅳ in youth was respectively higher than those of middle - aged patients[（33.4 ±15.3） % vs （23.3±17.0）%, and （3.0±6.9）% vs （1.1±2.8）%）]（P〈0.05）. Conclusion： Excluding the similar sleep structure disorder, the sleep disorder and low oxygen saturation of blood are more serious in youth than in middle-aged patients.
