变异念珠菌在血细胞内的发育与系统性红斑狼疮致病机制的初步研究

摘  要:目的观察血细胞内不同发育期的念珠菌变异体,了解念珠菌与系统性红斑狼疮致病机制的相互关系,为临床诊治提供依据。方法取患者外周血,制作薄血片固定,经Giemsa染色置光镜下观察;同时对样品用电镜固定液固定,包埋、超薄切片作透射电镜观察。结果光镜下观察到单核细胞胞浆中有纤丝样网状物质和微小的紫红色颗粒;小红细胞样生物体从红细胞内逸出;孢子样颗粒从孢子母细胞样生物体逸出。电镜观察到单核细胞内有微丝体、微棒状小体和空泡状发育期寄生物;红细胞崩解后,母细胞样、孢子母细胞样生物体从中释放出来;孢子在孢子母细胞内形成。经病原学检测,单核细胞和红细胞内的寄生物隶属念珠菌。结论单核细胞、红细胞可能是念珠菌的宿主细胞,胞内发育期的抗原成分能激活淋巴细胞,使其产生淋巴因子和抗体并存于组织中。当宿主细胞在某一组织和器官中集结,发生溃破时,释放出大量抗原物质,引起免疫反应,造成红斑狼疮的临床症状发生和病理性损害。To observe the developmental characteristics of Candida variant in blood corpuscles and its relation to the possible pathogenetic mechanism in systemic lupus erythematosus in order to provide its diagnostic and therapeutic basis, peripheral blood of patients was taken and thin blood smears were made, fixed and stained with Giemsa staining and observed under optical microscopy. Meanwhile, the tissue slices were observed by electron microscopy. It was found that under optical microscopy the fihrillar reticulations and the tiny purple-reddish granules in monocytes. Small red blood corpuscle-like organelles and sporule-like granules releasing from red blood corpuscles as well as the mother sporule cells could also be observed. Under electron microscopy, microfilament bodies, microrhoptryand vacuolar parasitic organisms at developmental stage were observed in monocytes. Mother cell-like and mother sporule cell-like organisms releasing from the broken red cells and the sporules growing to the mother sporule cells were observed. The parasitic ortganisms observed within red blood corpuscles and monocytes were proved to be the Candida species by etiological examinations. These obervations suggest that red blood cells and monocytes may be the host target cells for Candida species and the antigens releasing from different developmental stages of sporules may stimulate production of cytokines and antibodie in tissue, thus playing certain role in the pathogenesis of systemic lupus erythematosus.
