摘  要:目的研究核因子кB(NF-кB)介导的病毒基因反式激活途径在激素敏感型单纯性肾病(SRSNS)发病中的作用,探讨呼吸道病毒触发该病的机制。方法采用电泳迁移率改变分析、RT-PCR和ELISA法分别对SRSNS、肾炎性肾病、继发性肾小球疾病、毛细支气管炎和正常儿童外周血单个核细胞(PBMC)NF-кB转录活性、呼吸道病毒基因和抗体表达、血清IL-8水平进行检测。结果1SRSNS活动期NF-кB转录活性增高,与SRSNS恢复期、缓解期和其它组比较差异有统计学意义(P<0.05);2SRSNS活动期血清IL-8水平增高,与NF-кB转录活性呈直线正相关关系(r=0.88,P<0.001),NF-кB活性与呼吸道病毒检出阳性率亦呈正相关趋势。结论NF-кB介导的病毒基因反式激活途径能使IL-8等基因表达增高而致病,这可能是呼吸道病毒触发SRSNS的主要机制之一。Objective To investigate the path of viral transactivation of transcription by nuclear factorkappa B (NF-kB) in the pathogenesis of steroid-responsive simple nephrotic syndrome (SRSNS) triggered by respiratory tract viruses. Methods Children with SRSNS (including active stage, convalescent and remissive stage) were examined and were analytically compared to children with nephritic nephrosis, secondary glomerular diseases and bronchiolitis; healthy children were enrolled as controls. The methods of eleetrophoretic mobility shift assay (EMSA), of reverse transeription-PCR (RT-PCR), and of enzyme-linked immunosorbent assay (ELISA), were used to detect the transeriptive activity of NF-kB, the gene expression of respiratory tract viruses (respiratory syneytial virus-RSV, influenza virus-Flu) in peripheral blood mononuelear cells (PBMC), and the levels of IL-8 and viral antibodyies in serum, respectively. Results (1)Compared with the control and SRSNS in convalescent and remissive stage, the activity of NF-kB in PBMC of SRSNS in active stage was statistically elevated (P〈0. 05). (2)The level of IL-8 in serum of SRSNS in active stage was significantly increased, compared with healthy control(P〈0. 001). There was a positive linear correlation between the activity of NF-kB and the level of IL-8 in active stage of SRSNS, and a positive correlation trend was observed between the activity of NF-kB and the gene expression of respiratory tract viruses. Conclusion Viral transactivation of transcription by NF-kB may be an important path for respiratory tract viruses triggering the pathogensis of SRSNS.
