摘 要:目的探讨严重胸外伤患者加用胸腺因子治疗条件下白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平的变化。方法采用放射免疫法测定68例(A组)严重胸外伤者加用胸腺因子治疗与57例(B组)不加用者入院第2、4、7天血清IL-6、TNF-α的水平。另设30例健康人为C组。并对两组成人呼吸窘迫综合征(ARDS)发生率、感染率进行比较。结果与C组比较,A、B两组入院第2天IL-6、TNF-α明显升高(P<0.01);A组入院第4天较入院第2天IL-6、TNF-α明显降低(P<0.05),第7天较第4天下降更显著(P<0.05);B组入院第4天较第2天明显升高(P<0.05),第7天较第4天显著降低(P<0.05),但与第2天比较无显著性差异(P>0.05)。A、B两组ARDS发生率比较有显著性差异(P<0.05)。A、B两组感染发生率比较有显著性差异(P<0.05)。严重胸外伤者血清IL-6、TNF-α水平明显升高,第4天达最高峰。结论胸腺因子可能具有降低胸外伤者血清IL-6、TNF-α水平及ARDS、感染发生率的作用。Objective To study the level changes of (IL-6) and (TNF-α) of patients with severe thoracic trauma in the treatment added with thymic factor. Methods All the cases were divided into thymie factor treatment group (group A,68 cases) and group B(without thymic factor,57 cases),and healthy persons were taken as control (group C,30 cases).With the methods of radioimmunoassay the serum IL-6, TNF-α levels in the A and B groups were measured on the 2nd 4th,7th day after hospitalization.In addition,the incidence rates of ARDS and infection rates of A and B groups were compared.Results The serum IL-6,TNF-α levels of group A and group B were higher on the 2nd day after hospitalization than the group C (P〈0.01); the serum IL-6 and TNF-α levels of group A on the 4th day were significantly lower than that of on the 2nd day after hospitalization (P〈0.05), while it was lowest on the 7th (P〉0.05); the serum IL-6 and TNF-α levels of group B on the 4th day were significantly higher than that of on the 2nd day after hospitalization (P〈0.05), while it was lower on the 7th (P〉0.05), but on the 7th day there was not significantly difference than that of on the 2nd day. In the incidence rate of ARDS and infection rate there were significant differences between group A and group B (P〈0.05).Conelusion The levels of serum IL-6,TNF-α were significantly higher on severe thoracic trauma, were the highest on the 4th day. Thymie factor can effectively decrease the serum IL-6,TNF-α levels of severe thoracic trauma patients, and decrease the incidence rate of ARDS and infection rate.
