摘 要:目的探讨血液灌流(Hemoperfusion,HP)对急性甲胺磷中毒致呼吸衰竭的临床疗效。方法收集我院2000年6月~2004年9月应用HP治疗急性甲胺磷中毒伴呼吸衰竭病人5例,并用定量方法分析动态观察HP过程中患者血中甲胺磷浓度的变化及其临床疗效。结果HP前病人血中的平均甲胺磷浓度为1.65μg/ml,而HP结束后血中的甲胺磷浓度是0.49μg/ml;HP后45min血中甲胺磷浓度与HP前有显著性差异(P<0.05);同时病人的临床症状改善并脱离呼吸机全部存活。结论HP能清除体内的甲胺磷,对急性甲胺磷中毒伴呼吸衰竭可望提高生存率。Objective To investigate the ability of hemoperfusion to remove methylamine phosphate from the body and its clinical efficacy with respiratory failure. Methods Five cases of methylamine phosphate pesticide poisoning with respiratory failure in our hospital from June 2000 to September 2004 were studied,they were given hemoperfusion, Methylamine phosphate concentration changes in blood during hemoperfusion and the clinical efficacy were evaluated. Results The average concentration of methylamine phosphate in the body was 1.65μg/ml before the hemoperfusion and the average concentration was 0.49μg/ml in the end of hemoperfusion, the concentration was markly depressed after 45 minutes of the hemoperfusion treatment. Meanwhile all of the patients survived(P〈0.05): Conclusion Hemoperfasion could adsorb methylamine phosphate from the blood of methylamine phosphate pesticide poisoning,and it could possibly raise survival rale in methylamine phosphale pesticide poisoning with respiratory failure.
