摘  要:目的:研究耳声发射(OAE)筛查早产儿听力的基本特征,寻找适合早产儿听力筛查的方法、首次听力筛查的时间及评定标准。方法:应用丹麦产M adsen Cape ila耳声发射仪,对112例早产儿进行畸变产物耳声发射测试(DPOAE),测试结果不能通过者,1个月后复查,仍不能通过者3个月复查,如仍不能通过则同时行脑干诱发电位(ABR)检查,并对112例早产儿按窒息、体重及黄疸分组。结果:总通过率为86.6%,窒息组DPOAE通过率明显低于其他组,差异有显著性(P<0.05)。结论:DPOAE快速、方便、无损伤,可作为早产儿听力筛查的有效方法,且测试结果与窒息密切相关,胎龄34周是早产儿听力成熟的关键年龄,早产儿首次听力筛查的合适时间为胎龄34周,对第1次不能通过DPOAE的早产儿,尚不能断定为听力障碍,定期复查、随访很重要。Objective: To investigate the basic properties of otoacoustic emissions (OAE) in premature, search for a method of premature hearing screening, a optimal time and a evaluation criteria. Methods: Distortion product otoacoustic emissions (DPOAE) was used for hearing screening in 112 premature. The characteristics of DPOAE in premature were analyzed. Results: Pass rate was 86.6%. The pass rate of asphyxia group was very lower then that of study group. Conclusion: DPOAE is an objective, rapid and noninvasive technique, and it is the first choice of hearing screening in premature. 34 weeks of gestational age is a critical age for the auditory development. The first tested time of hearing screening in premature is 34 weeks of gestational age. Follow - up is very important.
