慢性乙型重型肝炎黄疸证患者外周血树突状细胞功能的研究

摘　　要：目的:比较慢性乙型重型肝炎阳黄证和阴黄证患者的树突状细胞功能。方法:体外培养上述病人外周血树突状细胞,流式细胞仪检测其表面分子CD80、CD86、CD40、HLA-DR、CD1a的表达,MLR中刺激T细胞的增殖和分泌IL-12、IFNα的能力。结果:阴黄证患者的DC表面分子CD86的表达较阳黄证组下降(P<0.05)。其他表面分子,如CD40、HLA-DR、CD1a和CD80,阳黄证与阴黄证两组无明显差异;经TNFα刺激后,上述各表面分子的表达较未加TNFα组均有上调,但阴黄证组CD80、CD86、HLA-DR上调的幅度低于阳黄证组(P<0.05)。阴黄证患者组自身T淋巴细胞增殖的能力较阳黄证患者组为低,但差异无显著性意义(P>0.05);经DC刺激后,阴黄证患者的T淋巴细胞的增殖效果明显低于阳黄证组(P<0.05)。阳黄证和阴黄证患者的IL-12和IFNα水平未经TNTα刺激前无明显差异,经TNTα刺激后两组的IL-12和IFNα水平较同组未经TNTα刺激者明显升高(P<0.05)。但阴黄证组上升的幅度低于阳黄证组(P<0.05)。结论:慢性乙型重型肝炎阳黄证和阴黄证患者均存在明显外周血DC功能障碍,运用TNFα激活可提高DC的部分功能,阴黄证患者的外周血DC细胞功能低下较阳黄证患者更为明显。Objective ：To compare the phenotypy and function of dendritic cells（DCs） in patients with two sub - type of TCM of chronic severe hepatitis Bo Method： 10 patients with chronic severe hepatitis were divided into 2 groups according to the charactefite of their icterus based on TCM （ Yang - icterus group and Ying - icterus group）, peripheral blood monocytes （PBMCs） were isolated from those patients. DCs were generated by culturing PBMCs in 1000 u/ml granulocyte - macrophage stimulating factor（ GM - CSF） and 500 u/ml IL -4 with serum -free AIM - V in vitro. After 8 dayincubation, the phenotypic patterns of DCs were characterized by flow cytometry and the level of IL - 12 and IFNα produced by DCs were analyzed by ELISA,Autologous T cells proliferation stimulated by DCs was tested by non -radioactive cell proliferation assay kit. Result：The expression rates of CD86 in Ying - icterus group was （48±10） % ,significantly lower than that of the Yang -icterus group;there is no significant difference between two groups on the expression rates of other phenotype（ CD40/CD80/CD1a/HLA - DR）. the expression rate of the phenotype in DCs pulsed with TNFα were higher,but the length of CD80, CD86, HLA - DR in Ying - icterus group is lower than those of the Yang - icterus group. The stimulatory capacity of DCs pulsed TNFα to induce proliferation of autologous T cells in Ying - icterus group is less powerful than that of Yang - icterus group （ P 〈 0.05 ）. The levels of IL - 12 and IFNα produced by the DCs pulsed with TNFα in Yang - ietems group were（498±120） and（87±7 ） respectively,both significantly higher than those in Ying - ieterus group. Conclusion ： The defective function and immature phenotype of DCs exist in patients with chronic severe hepatitis B. It＇s function could recovered partly pulsed with TNFα. Functions of DCs in patients with Ying- icterus is less powerful than those with Yang- icterus.
