摘  要:目的探讨女性充溢性尿失禁上尿路扩张患者尿动力学参数的变化。方法回顾性分析55例女性充溢性尿失禁患者的尿动力学检查结果。按照上尿路情况分为上尿路扩张组(31例)和无上尿路扩张组(24例)。结果上尿路扩张组患者排尿后残余尿量、最大膀胱压测定容量、逼尿肌漏尿点压分别为(370.3±242.5)ml,(534.0±231.4)ml,(3.4±2.3)kPa,分别高于无上尿路扩张组的(29.5±21.1)ml,(366±201)ml和(2.0±1.4)kPa;扩张组膀胱顺应性为(141.8±56.1)ml/kPa,低于无扩张组的(336.7±154.1)ml/kPa,P<0.05。扩张组膀胱危险容量、危险系数较非扩张组增高,P<0.05。结论尿动力学检查可以准确评估女性充溢性尿失禁患者膀胱功能状态,为预防上尿路损害和临床治疗提供客观依据。Aim : To observe the urodynamic changes in female patients with overflow incontinence complicated with upper urinary tract dilatation (UUTD). Methods: Fifty-five patients with overflow incontinence were included in this study. They were divided into two groups according to change of upper urinary tract by ultrasound : Group Ⅰ including 31 patients with UUTD, Group Ⅱ including 24 patients without UUTD. Results: In group Ⅰ,the post void residual, maximum cystometric capacity, detrusor leak point pressure were ( 370.3 ± 242.5 ) ml, ( 534.0 ± 231.4 ) ml, and ( 3.4 ± 2.3 ) kPa, respectively, higher than those in group Ⅱ (29.5 ±21.1 ) ml, (366 ±201 ) ml, and (2.0 ± 1.4) kPa. Bladder compliance in group Ⅰ was ( 141.8 ± 56. 1 ) ml/kPa, significantly lower than that of group Ⅱ (336.7 ± 154. 1 ) mL/kPa ( P 〈 0.05 ). Relative unsafe capacity and relative capacitied risk rate of group Ⅰ were significantly higher than those of group Ⅱ (P 〈 0.05). Conclusion: Urodynamics examination is a useful tool to predict the risk of UUTD in female with overflow incontinence.
