Gore-Tex人工腱索在二尖瓣前叶脱垂修复术中的应用

摘　　要：目的评价使用Gore-Tex作为人工腱索治疗二尖瓣前叶脱垂的近、远期疗效。方法比较38例使用Gore-Tex作为人工腱索治疗二尖瓣前叶脱垂病人术前、术后心功能、左心室、左心房直径和二尖瓣反流程度的变化。结果病人术后心腔明显缩小。左房直径从术前(56·24±5·98)mm减少至(37·58±5·64)mm(P<0·05),左室舒张末直径从术前(64·17±8·25)mm减少至(50·88±5·34)mm(P<0·05),左室收缩末直径从术前(49·13±6·02)mm减少至(38·11±5·01)mm(P<0·05)。术后心功能较术前明显改善,射血分数(EF)从术前的0·58±0·08提高至0·64±0·06(P<0·05)。术后二尖瓣反流程度明显减轻,术前(3·76±0·43)+,术中食管超声(0·76±0·68)+,随访二尖瓣反流程度为(1·08±0·99)+,(P<0·05)。结论Gore-Tex线作为人工腱索替代物安全、可靠,中期疗效满意。Objective To evaluate the clinical application and mid-term result of Gore-Tex suture as an artificial chontae in mitral anterior leaflet valvuloplasty. Methods 38 patients with the anterior leaflet received surgery. The data of the cardiac function （EF）, the divmeter of left ventricular and left atria and the degree of mitral regurgitation were analyzed before and after operation. Results The degree of mitral regurgitation decreased significantly after operation. The mean degree of pre-operation mltral regurgitation was （ 3.76 ± 0.43） ＋ , and after surgery was （ 1.08 ± 0.99 ） ＋ , P 〈 0.05. The diameter of left ventrieular and left atria reduced postoperatively. The posture eardiac function improved significantly. EF increased from 0.58±0.08 preoperatively to 0.64 ± 0.06 postoperatively （ P 〈 0.05）. The Gore-tex suture as an artificial chordae is a safe and effective technique in mitral anterior leaflet valvuloplassty.
