糖尿病患者肾小球滤过率测定的临床意义

摘　　要：目的：观察糖尿病不同时期的肾小球滤过率（ＧＦＲ），方法：用￣(９９ｍ)ｃ－ＤＴＰＡ测定３０例正常人、２０例初发Ⅰ型糖尿病和１３９例Ⅱ型糖尿病者的ＧＦＲ，并检测尿白蛋白排泄率（ｕＡＥＲ）。结果：初发Ⅰ型糖尿病（ＪＦＲ（１２３±８．６ｍｌ／ｍｉｎ）高于正常组（１０６±７．２ｍｌ／ｍｉｎ）．Ｐ＜０．０１；Ⅱ型糖尿病随ｕＡＥＲ增加，ＧＦＲ逐渐下降，二者呈负相关（ｒ＝０．９１，Ｐ＜０．０１）。结论：糖尿病早期存在肾小球高滤过，并用ＧＦＲ和ｕＡＥＲ检测对早期发现和治疗糖尿病肾病有重要意义。Objective:To observe glomerular filtration rate(GFR)in different stages of diabetic nephropathy.Methods:Using￣(99m)Te-DTPA,we measured the GFR of 30 normal people,20 pa-tients newly diagnosed as having typeⅠdiabetes mellitus and 139 patients with typeⅡdiabetes melli-tus; and detected their urinary albumin exeretion rate (uAER).Results:The GFR of patients with typeⅠ diabetes mellitus(123±8.6 ml/min ) was higher than that of normalsubjects (106±7.2 ml/min)P<0.01).In type Ⅱ dinhetes mellitus, with the inerease of uAER,the GFR gradually declined,and as correlated with uAER(r=-0.91 P<0.01).Conclusions:In early stage of diabetes mellitus the glomerular was in a condition of hyperfiltration,The combined measurement of GFR with uAER was of important value in early finding and treating the patients predisposed to diabetic nephropathy,
