摘  要:目的观察3日龄SD大鼠慢性缺血脑损伤后脑室下区细胞凋亡情况,了解早产儿缺血后脑进一步损伤的发病机理,为干预治疗提供时间窗实验依据。方法3日龄新生SD大鼠随机分为对照组和实验组。实验组结扎双侧颈总动脉,造成未成熟鼠慢性脑缺血脑损伤模型;对照组双侧颈总动脉不结扎.组织切片HE染色定位,采用TUNEL法分别观察脑室下区背侧、前端细胞凋亡变化。结果脑室下区背侧、前端凋亡细胞分别计数发现在缺血48、72h和1周后较对照组增多(P〈0.01),于72h时间点两组差异最为显著(背侧:实验组87.0±11.5,对照组60.9±7.1;前端(冠状位):实验组86.1±5.5,对照组56.4±5.6;前端(矢状位):实验组69.1±3.5,对照组44.0±2.3,P〈0.01),损伤后2周两组差异无统计学意义(P〉0.05)。结论3日龄SD大鼠慢性脑缺血后脑白质损伤外,神经生发区之一;脑室下区细胞对缺氧缺血敏感,易发生凋亡,其可能为早产儿脑白质进一步损伤的机制之一,同时此模型提示慢性缺血缺氧可致神经元再生不足,提示缺血后48~72h可能为防止脑室下区损伤的时间窗。Purpose To understand the mechanism of progressive premature brain injury after ischemia and to provide the experiment evidence of treating time window. Methods Eighty 3-day-old rats were divided into 2 groups:controlling group and experiment group. Rats in experimenting group were subjected to bilateral common arteries occlusion; rats in controlling group were not. We observed and calculated the apoptotic cells in the dorsolateral subventricular zone(SVZdl) and the anterior subventricular zone(SVZa). Results At the time point of 48 hour,72 hour, 1 week of postischemia in the rats of experiment group, the numbers of the apoptotic cells in the SVZdl and SVZa were more than those in the controlling group. The numbers of the apoptotic cells at the time point of 72 hour of postischemia were the most prominent[SVZdl : (87.0 ± 11.5) vs (60.9 ± 7. 1 )] ;[ SVZa (coronal axis) : (86. 1 ± 5.5) vs (56. 4 ± 5.6) ;SVZa(sagittal axis)(69.1 ± 3.5) vs (44. 0 ± 2. 3) ,P〈0.01]. Conclusions The cells in the subventricular zone are susceptive to ischemia, which may result in the less regenerating cells in white matter; 48-72 hour of postischemia in the premature rats may be the treating time window.
