肝移植术后肝癌复发的超声表现

摘　　要：目的探讨肝移植术后复发的超声表现及术前超声检查应注意的情况,作为确定肝移植术适应症的参考。方法对2004.1-2005.7本院6例原发性肝癌肝移植术后复发病人,行常规超声检查,并回顾术前患者病史。结果6例肝移植患者中,3例于术后半月、四个月、半年发现肝内复发灶,其中2例同时伴有肝外转移灶;另3例于术后一月、一个半月、半年发现肝外转移灶。结论肝移植术后肝癌复发、肝外转移并非少见,故术后及早、密切、定期行超声检查,对早期发现及早处理是极为必要的。术前超声检查有助于临床更好地掌握肝移植术的指征,及较准确估计肝移植预后。Purpose To discuss the ultrasonic performances of the recurrence of the hepatocelluar carcinoma （HCC） after liver transplantation and the matters should be concerned in the ultrasonic examination before the operation. The results of which might be deemed as the reference of confirming the indication of liver transplantation. Methods Examining the six cases in our hospital from January 2004 to July 2005 in which the patients recurred after the HCC liver transplantation operation. Analyzing the medical history of the patients before the operation. Results Three of the six patients developed recurrent focus inside liver after a half of month,four months and a half of year,two of which had metastatic focus out of liver simultaneously. Other three patients developed metastatic focus out of liver after one months,one and a half months and a half year. Conclusion As the recurrence of liver cancer and the metastasis out of liver are regular, it is necessary to examine the patients promptly, frequently and regularly by ultrasound so that such recurrences and metastasis can be found and dealt with as early as possible. The ultrasonic examination before the operation will be beneficial for clinic both to have a better understanding of the indication of the liver transplantation survey and to estimate the prognosis hereof more accurately.
