摘  要:目的:探讨百令胶囊对稳定期慢性阻塞性肺病(chronic obstructive pulmonary disease,COPD)患者血清TNF-α、IL-8、GM-CSF水平及FVC、FEV1、FEV1/FVC的影响。方法:对照组、患病组行肺功能中FVC、FEV1检测(包括支气管扩张试验)。采用ELISA方法测定各组血清中TNF-α、IL-8、GM-CSF水平。用纤维支气管镜取患病组肺组织,在光镜下观察。治疗组口服百令胶囊30d后用同样的方法测定上述指标。结果:患病组血清TNF-α、IL-8、GM-CSF的水平较对照组高,患病Ⅰ组、Ⅱ组、Ⅲ组随分级的增加而增高(P<0.05)。Ⅲ组患者细胞因子水平最高,分别是(28.46±9.47)pg/mL、(100.37±4.90)pg/mL、(146.00±18.85)pg/mL。治疗后TNF-α、IL-8、GM-CSF在血清水平明显下降(P<0.05),而FVC、FEV1及FVE1/FVC在治疗后有明显的升高(P<0.05)。光镜下观察治疗前后肺组织纤维增生无明显改变。结论:慢性阻塞性肺病患者血清中的细胞因子水平高于健康对照组,在不同级别存在差异(P<0.05),分级越高因子水平越高。百令胶囊可降低COPD血清中增高的细胞因子水平,进而改善COPD患者的肺功能。Objective: To investigate the effects of Bailing capsule on serum levels of TNF-α, IL-8, GM-CSF and pulmonary functions in patients with chronic obstructive pulmonary disease (COPD). Methods: The determinations of FEV1,FVC including bronchodilation test were performed for both control and illness groups. The levels of TNF-α,IL-8,GM-CSF in serum were tested by ELISA. Pulmonary tissues were taken by fiberbronchialtroscope and observed by Optic Microscope. The same parameters were checked after taking bailing capsule for 30 days. Results: The level of TNF-α, GM- CSF,IL-8 level in patients were higher than in control, and these cell factors level were gradually high with the grade(P〈0.05), TNF-α,GM-CSF,IL-8 level in Ⅲ group were higher than in two other groups, respectively (28.46±9.47) pg/mL, (100.37±4.90) pg/mL, (146±18.85) pg/mL. The amount of FVC, FEV1 in every group were significantly improved with the cell factors level decreased in serum after being treated by taking bailingjiaonang (P〈0.05). The lung tissues were not changed under optical microscope after being treated. Conclusions: The cell factors level in COPD patients were higher than in control, and there were different for the cell factors level in COPD every group (P〈0.05), The level of cell factors in Ⅲ group were highest, Ⅱ group were second, Ⅰ group were final. Bailingjiaonang may decrease the level of TNF-α,GM-CSF,IL-8 in serum, as well as improve significantly the lung function of COPD.
