纤支镜代胸腔镜治疗老年慢性阻塞性肺气肿并发难治性气胸

摘　　要：【目的】探讨经纤支镜代胸腔镜注入白蛋白治疗老年慢性阻塞性肺气肿并发难治性气胸的价值。【方法】常规闭式引流术未愈后,在局麻下以纤支镜代胸腔镜对胸膜腔进行观察,对较小的粘连带进行离断,并局部应用白蛋白对漏气口进行治疗,观察预后。【结果】21例中20例治愈。【结论】采用纤支镜代胸腔镜,通过对粘连带离断以及局部应用白蛋白,治疗老年慢性阻塞性肺气肿并发难治性气胸,确实有效,且该法操作简单、安全,病人痛苦少、费用低,尤其适用于年老体弱、心肺功能差的病人,具有较好的应用前景,而且符合微创要求。[Objective]To evaluate the value and safety of treating elder chronic obstructive pulmonary emphysema compliated obstinate pneumothorax with human albumin via fiberbronchoscopy. I[Methods]Twentyone patients with obstinate pneumothorax were undergone video assisted fiberbronchoscopy after routine close drainage of thoracic cavity. Adhesive band was disconnected with biopsy forceps or microwave. Human albumin was injected to the bullae or smeared on the surface of the lung. The efficacy and side effects were observed. I[Results]Twenty patients were cured. The total cure rate was 95.23%. I[Conclusion]This method is not only useful,simple and safe, but also with little pain and low costs. This minimally invasive surgical intervention is especially applicable to the elder physically weak patients with feeble heart and lung. So it has good prospect.
