摘  要:目的探讨糖皮质激素对脓毒症大鼠肾细胞凋亡的影响。方法将30只大鼠随机分为对照组、脓毒症组和地塞米松组,每组10只。其中脓毒症组和地塞米松组采用盲肠结扎穿孔术(CLP)致脓毒症模型,地塞米松组行盲肠结扎穿孔术后即给予地塞米松,测定各组大鼠肾功能及肾细胞凋亡的情况。结果与对照组相比,CLP术后血清尿素氮、肌酐水平及肾脏细胞凋亡指数明显升高(P<0.01);地塞米松组与脓毒症组比较血清尿素氮、肌酐水平以及肾脏细胞凋亡指数均降低(P<0.01)。结论在CLP所致的脓毒症早期,应用地塞米松对肾脏有一定的保护作用,其作用机制可能与其抑制肾细胞凋亡的发生有关。Objective To explore the effect of glucocorticoid hormone on apoptosis of renal cells of septic rats. Methods Thirty-six rats were randomly divided into three groups, control group, septic group and a dexamethasone(Dex)-treated group. The septic model was indieed by cecal ligation and puneure (CLP)in septic group and Dex-treated group. The serum levels of BUN and Cr were measured. At the same time, the number of renal cells apoptosis was also measured. Results The levels of BUN and Cr at 5h after CLP showed a notable discrepancy eompared to control group(P 〈 0.01 ). The levels of BUN and Cr in Dex-treated group after CLP showed a notable discrepancy compared with the septic group(P 〈 0.01). The number of renal cells apoptosis in was higher than that in control group(P〈0.01),and the number in Dxa treated-group at 5h showed a notable discrepancy compared to septic group(P〈 0.01 ). The changes of ultra structure in renal in Dxa treated-group were slightly under election mieroscope. Conclusion During the early period of CLP ,Dxa had a preventive role on renal function, which may suppress the renal apoptosis.
