摘　　要：目的评价国产EXCEL雷帕霉素药物洗脱支架临床疗效。方法90例冠心病患者分3组,分别植入EX-CEL药物洗脱支架、Cypher药物洗脱支架及普通金属支架共计195枚,观察了3组支架的安全性及临床近期疗效。结果3组狭窄程度及病变长度无明显差别,分别经(12.9±2.1)atm、(13.4±3.3)atm、(13.0±4.2)atm扩张支架膨胀完全,均可完成对吻扩张,总的植入成功率为99.5%。与普通支架相比,使用EXCEL及Cypher药物洗脱支架组患者年龄大,合并高血压等心血管疾病数多(均为P<0.05)。使用EXCEL支架组3支病变也明显多于Cypher及普通支架组(P<0.05)。75例患者随访超过3个月(随访率为83.3%),普通支架组1例猝死,MACE发生率为2.7%,心绞痛复发4例;两种药物洗脱支架随访无1例发生MACE,心绞痛复发5例。结论国产EXCEL雷帕霉素药物洗脱支架临床应用安全,近期疗效与Cypher药物洗脱支架相近,PC I后近期主要不良心脏事件的发生率明显低于普通金属支架。Objective To evaluate the clinical effects of homemade EXCEL drug-eluting stents. Methods Ninety patients with coronary diseases were divided into three groups. EXCEL sirolimus coated stents, Cypher drug-eluting stents, and bare metal stents were implanted separately in the three groups. The total number of stents was 195. The safety and efficacy of the three types of stents were observed. Results There were no differences in stricture degree and length of lesions among the three groups. All the stents were fully dilated by （ 12.9 ± 2.1 ） arm, （ 13.4 ± 3.3 ） arm, and（ 13.0 ± 4.2 ） arm respectively. The total rate of success stent implantation was 99.5%. However, the patients using the EXCEL drug-eluting stents or Cypher drug-eluting stents were significantly elder and experienced more complications （ e. g. hypertension） than those using bare metal stents （ P 〈 0.05）. Patients in the EXCEL drug-eluting stents group experienced more triple vessel disease than those in the other two groups（ P 〈0.05 ）. Seventy-five cases were followed up for more than 3 months and the follow up rate was 83.3 %. In the bare metal stent group ： there was one sudden death; the MACE rate during following up period was 2.7% ;4 cases of recurred angina. In the two drug-eluting stent groups： there was no case of MACE occurrence; there were 5 cases of recurred angina. Conclusion EXCEL drug-eluting stent is safe and effective in clinical application. It could provide similar outcomes as Cypher drug-eluting stents. The MACE rate could be reduced obviously comparing with bare metal stents after PCI in the near future.
