摘 要:目的评估呼吸功能训练辅助治疗吸入性损伤患者的效果。方法选取近3年来收治的烧伤面积小于50%全身体表面积、合并吸入性损伤的患者40例,根据吸入性损伤程度随机分为训练组(n=20)、对照组(n=20),两组患者均按常规综合治疗,对训练组加予腹式缩唇呼吸、有效咳嗽排痰及吸气训练器训练,观察记录两组患者气管套管拔除管时间及肺功能改善情况。结果训练组患者拔管时间显著提前,最大通气量、脉搏氧饱和度、第1秒肺活量、氧分压均有不同程度提高。结论呼吸功能训练辅助治疗吸入性损伤可缩短气管套管留置时间,改善患者肺功能。烧伤科护理人员需重视对吸入性损伤患者进行呼吸功能训练,掌握时机与时限,以增进疗效,增强患者自我效能。Objective To evaluate the effects of inhalation injury patients treated with respiratory function training. Methods Forty patients (TBSA〈50%) with inhalation injury who were enrolled in last 3 years were divided into training group (n=20) and control group (n=20) according to the injury degree. All the patients received routine therapy,and the training group was added withdraw-lip abdominal respiratory exercises, effective cough and expectoration volumetric exercises. The removing time of tracheal tube, rate of pulmonary infection and improvement of pulmonary function were observed and recorded. Results The removing time of tracheal tube, the rate of pulmonary infection, the improvement of Maximal Voluntary Ventilation (MVV), arterial blood oxygen saturation (SaO2), forced expiratory volume (FEV 1.0) and partial pressure of oxygen (PaO2) were improved to a certain extend in the training group. Conclusion Respiratory function training can shorten the removing time of tracheal tube, improve the pulmonary function and decease the pulmonary infection rate. Nurses of burning department must pay attention to the respiratory function training for patients with inhalation injury so as to improve therapeutic effect and enhance self-efficacy.
