尿微量蛋白在糖尿病早期肾脏损伤诊断中的作用

摘  要:目的探讨尿微量蛋白(M A)检测对早期肾脏损伤的诊断价值。方法M A、1α微球蛋白(1α-M)、转铁蛋白(TRF)、IgG测定用速率散射比浊法;尿蛋白定性测定用尿11联试纸。结果糖尿病组与对照组比较,尿蛋白定性、尿M A、1α-M、TRF、IgG检测结果有显著性差异。初期肾病组的尿M A、1α-M、TRF阳性率相近,明显高于尿蛋白定性、尿IgG。临床肾病组的尿蛋白定性、尿M A、1α-M、TRF检测阳性率无显著性差异。结论尿M A,1α-M,TRF是诊断早期肾脏损伤的灵敏的指标。Objective To investigate the diagnostic value of urine microproteins in early kidney damage. Methods MA,α1-M, TRF and IgG in urine were detected with rate nephelometry and protein in urine was qualitatively measured with uropaper. Results Concentration of MA,α1-M,TRF and IgG in urine was significantly higher in diabetes group than in control group. Positive rate of MA ,α1-M and TRF was not obviously different, but obviously higher than that of qualitative urine protein and IgG in the diagnosis of the early nephropathy group. Qualitative urine protein,urine MA,α1-M and TRF were not statistically different in clinical nephropathy group. Conclusion Urine MA,α1-M,TRF in urine are sensitive indexes in diagnosis of kidney damage.
