摘  要:目的:探讨畸变产物耳声发射(DPOAE)对伪聋和夸大性聋的鉴别诊断作用。方法:采用ILO 96耳动态分析仪对外伤后诉听力下降,而又不配合纯音听阈(PTT)检测的患者进行DPOAE检测与分析。结果:经PTT检测的129例(150耳)外伤性听力下降患者中,有102例(121耳),作DPOAE检测结果显示伪聋占66.12%(80/121),夸大性聋占33.88%(41/121)。经ABR检测反应阈值,证实DPOAE检测结果客观、可靠。结论:DPOAE作为外伤后听力损失鉴别诊断的常规检测方法,可对伪聋和夸大性聋作出客观、可靠的诊断和鉴别诊断。Objective:To explore the role of differential diagnosis of distortion product otoacoustic emissions (DPOAE) in pseudohypacusis and exaggerated hearing loss. Method:Detection and analysis of DPOAE were made by ILO96 Otodynamic Analyzer in the patients complaininged of post-traumatic hearing loss and incooperative in pure tone threshold ( PTT ) examination. Result:The detective results of DPOAE in 102 patients (121 ears ) incooperative in PTT examination revealed that pseudohypacusis was 66.12% ( 80/121 ears ), exaggerated hearing loss was 33. 88% ( 41/121 ears ). The reactive threshold value determined by auditory brainstem response (ABR) in these patients confirmed that the results of DPOAE were objective and reliable. Conclusion:DPOAE can be used as a routine examination method in differential diagnosis of the post-traumatic hearing loss. It gives an obiective and reliable diagnosis and differential diagnosis for the pseudohypacusis and exaggerated hearing loss.
