摘 要:目的探讨相位对比法MRI测量中心肺动脉各血管的血流速度和血流量的准确性以及临床应用价值。方法正常健康志愿者21例,采用相位对比法MRI测量中心肺动脉,包括主肺动脉、右肺动脉、左肺动脉、右肺上叶动脉、右肺叶间动脉和左肺叶间动脉各血管的直径、一个心动周期的最大血流速度、平均速度和瞬时流量,并计算出各血管一个心动周期的血流量。结果①中心肺动脉各血管的直径分别为(24.5±3.4)mm、(16.9±2.6)mm、(16.3±1.8)mm、(6.1±1.3)mm、(12.0±1.6)mm和(11.3±1.4)mm。②中心肺动脉各血管的最大血流速度分别为(79.9±24.0)cm/s、(78.7±20.2)cm/s、(77.4±22.1)cm/s、(66.1±9.2)cm/s、(73.5±19.6)cm/s和(72.1±16.4)cm/s。③中心肺动脉各血管的平均速度分别为(50.3±7.7)cm/s、(49.9±11.0)cm/s、(46.6±9.7)cm/s、(38.9±8.9)cm/s、(44.4±7.7)cm/s和(43.0±8.2)cm/s。④中心肺动脉各血管一个心动周期的血流量分别为(73.6±6.6)ml、(38.9±4.5)ml、(33.7±3.2)ml、(12.7±5.9)ml、(29.0±10.9)ml和(19.9±7.0)ml。⑤中心肺动脉的直径和血流量的对应点分布比较集中,呈现出直线上升的趋势,可以虚拟出高度相关的回归直线和直线相关方程,即Y=3.06X-7.04,相关系数r=0.90(P<0.001)。结论相位对比法MRI可以准确测量中心肺动脉的血流速度和血流量,为无创性肺动脉血流动力学的研究奠定基础。Objective To assess the accuracy and application values of phase-contrast MRI by measuring the flow velocity and volumes of the central pulmonary arteries. Methods Diameter, maximum blood flow velocity, mean blood flow velocity and flow volume rate in a cardiac cycle of central pulmonary arteries in twenty-one healthy volunteers were measured at a 1.5T MR scanner. The blood flow volumes in a cardiac cycle were calculated according to formula F= ∑ (ml/s)×(s/ frames). The central pulmonary artery included the main pulmonary artery, the right pulmonary artery, the left pulmonary artery, the right upper artery, the right descending artery and the left descending artery. Results ①The diameters of central pulmonary artery were (24.5±3.4) mm, (16.9±2.6) mm, (16.3±1.8) mm, (6. 1±1.3) mm, (12. 0±1. 6) mm, and (11.3±1.4) mm respectively. ②The maximum blood flow velocities were (79.9±24. 0) cm/s, (78. 7±20. 2) cm/s, (77. 4±22.1) cm/s, (66.1±9.2) cm/s, (73. 5±19.6) cm/s, and (72. 1±16.4) cm/s respectively.③The mean blood flow velocities were (50.3±7.7) cm/s, (49.9±11.0) cm/s, (46.6±9.7) cm/s, (38. 9±8. 9) cm/s, (44.4±7. 7) cm/s, and (43.0±8. 2) cm/s respectively. ④The flow volumes of central pulmonary artery in a cardiac cycle were (73.6±6. 6) ml, (38. 9±4.5) ml, (33.7±3. 2) ml, (12.7±5. 9) ml, (29.0±10. 9) ml, and (19.9±7.0) ml respectively. ⑤The corresponding dots of diameters and flow volumes of central pulmonary artery were centralizing and presented ascending linear current. The regression line was drafted and the linear correlation-regression formula Y= 3.06 X-7.04 was calculated, correlation coefficient was 0. 90 (P〈0. 001 ). Conclusion The blood flow velocity and flow volume of the central pulmonary artery could be measured accurately with phase-contrast MRI, which establish good base for the noninvasive study of pulmonary artery hemodynamics.
