老年人消化性溃疡病合并大出血的临床特点及治疗探讨(附139例分析)

摘　　要：收集20年间医院收治的老年性溃疡病合并上消化道大出血139例。所有病例都经内镜及手术证实。平均发病年龄66.7岁,有明确溃疡病史者80.6%(112/139),出血时伴有其它合并症者占40.3%(56/139)。手术止血73例(52.5%),手术死亡10例,病死率13.7%)。对老年人溃疡病合并上消化道大出血的发病特点,及临床处理原则进行了较深入的探讨。认为老年人患本病因其合并症多,病死率高,在临床处理上应在治疗合并症,维护心、肺、肾等脏器功能的同时,积极采取必要的手术治疗。This paper reported 139 aged patients who suffered from peptic ulcer with acute massive haemorrhage in our hospital during last 20 years. All 139 cases were confirmed by gastroscopy and surgical operation. The average age of the patients was 66.7 years old. Of 139 patients, 112 cases had the history of peptic ulcer, 56 cases were accompanied with other complications when haemorrhaging, 73 cases underwent surgical operation. Operational mortality was 13.7 per cent. The clinical characteristic and treatment principie. of the aged patients suffered from peptic ulcer complicated with acute massive haemorrhage were discussed in this paper. The authers realized that it is important to take positive treatment measures by comparatively loosening surgical operational indication while curing the complication and take care in keeping the funcions of the main organs such as heart, lungs and kidneys.
