摘　　要：目的探讨应用放射性核素99mTc-MAA(大颗粒聚合白蛋白)行深静脉造影(RNV)的最佳注射方法,以获得高质量的RNV图像。方法通过双下肢足背静脉3种不同给药方法:一次性“弹丸式”推注全部药量、缓慢等速推注、分3段推注药,对3种方式进行造影效果比较。采用膝下用血压计袖带加压至8.0kPa(60mmHg)和双踝关节上方扎止血带加压2种不同方法对双下肢浅静脉进行加压观察造影效果。结果一次性“弹丸式”给药方法的RNV图像清晰率为54%,缓慢等速推注药RNV图像清晰率达90%,分3段推注药RNV图像清晰率达93%;同时应用止血带加压方法显像成功率达96%,显著高于血压计袖带加压法(81%)。结论缓慢等速推注药和分3段推注药均比一次性“弹丸”给药方法RNV图像质量好,分3段推注药方法最佳。止血带加压方法简单而且灵活,如果首次加压失败,立刻调整止血带改变肢体内侧压力,可明显提高显像成功率。Objective To explore a best method of injecting ^99mTc-MAA for radionuclide deep vein imaging （RNV） in order to obtain high quality RNV images. Methods Three methods to inject ^99mTc-MAA through veins in the dorsum of foot were introduced： onetime injection of the full dosage, slow injection at an equal speed and injection at three times. The images obtained at the three different methods of injection were compared. On the other hand, the effects of RNV imaging under two different ways： pressing to 8.0 kPa （60 mmHg） under the knees with a hemomanometer cuff and pressing above ankle joint with prelum arteriol were compared as well. Results The sharpness rates of RNV images in the three methods： one-time injection of the full dosage, slow injection at an equal speed and injection at three times, were 54%, 90% and 93%, respectively. On the other hand, the success rates of RNV imaging at the different methods： pressing to 8.0 kPa （60 mmHg） under the knees with a hemomanometer cuff and pressing above ankle joint with prelum arteriol, were 81% and 96%, respectively, with significant difference statistically. Conclusion Slow injection at an equal speed and injection at three times can both result in better images, compared with the method of one-time injection of the full dosage and the last method is the best. During RNV imaging, pressing above ankle joint with prelum arteriol is simple and flexible. If it fails for the first time, timely adjustment of the prelum arteriol to change the inside pressure of the extremities may improve the success rate of imaging.
