维持血液透析患者微炎症状态相关因子与低血清白蛋白血症的关系

摘　　要：目的探讨维持血液透析(MHD)患者微炎症状态相关因子[血清中肿瘤坏死因子(TNFα)、可溶性肿瘤坏死因子受体(sTNFR)和白细胞介素6(IL6)]与低血清白蛋白(ALB)血症的关系。方法选择39例MHD患者,并根据ALB水平分为2组:ALB<35g/L组(27例)和ALB≥35g/L组(12例)。应用双抗体夹心酶联免疫吸附法(ELISA)检测血清TNFα、sTNFR和IL6水平,与30例健康体检者(对照组)进行对照。结果MHD患者TNFα、sTNFR和IL6水平高于对照组,且ALB<35g/L组TNFα、sTNFR和IL6水平较ALB≥35g/L组升高更为明显,组间比较有统计学意义(P<0.05);ALB与TNFα、sTNFR及IL6浓度呈负相关。结论MHD患者血清TNFα、sTNFR和IL6水平升高,存在微炎症状态。低ALB血症与高TNFα、sTNFR及IL6浓度相关。Objective To evaluate the relationship between microinflammatory state related-factors, the tumor necrotizing factor （TNF-α）,soluble tumor necrotizing factor receptor（sTNF-R）, interleukin-6（IL-6）, and hypoalbuminemia in maintenance hemodialysis patients. Methods 39 patients undergoing MHD were selected and classified into two groups according to their albumin（ALB） levels： the group with ALB〈35 g/L（27 cases） and that with ALB≥35 g/L（12 cases）,and 30 healthy persons were selected as control group. TNF-α, sTNF-R and IL-6 were measured with enzyme-linked immunosorbent assay （ELISA）. Results the levels of TNF-α, sTNF-R and IL-6 were increased significantly in MHD group compared with control group, and were higher in ALB〈35 g/L group than ALB≥35 g/L group also（P〈 0.05）. Hypoalbuminemia were negatively correlated with TNF-α, sTNF-R and IL-6. Conclusion TNF-α, sTNF-R and IL-6 increased in MHD patients, which means the microinflammatory state is present in MHD patients. Hypoalbuminemia is associated with the increased of TNF-α, sTNF-R and IL-6 level.
