摘 要:目的:研究应用弧形交叉髓内钉在治疗指骨骨折的固定可行性。方法:在12例新鲜尸体手指标本上,模拟横断骨折,分别采用弧形交叉髓内钉与普通交叉克氏针进行固定,以MTS试验机模拟持续被动运动(CPM)进行屈伸疲劳,检测固定效果。结果:疲劳前后在水平方向上骨折对位精度弧形交叉髓内钉固定明显高于普通交叉克氏针固定(P<0.05),在矢状和垂直方向上两者无显著差异;除垂直方向外,弧形交叉髓内钉固定在疲劳前、后的比较无差异(P>0.05),而普通交叉克氏针存在显著性差异(P<0.05)。结论:弧形交叉髓内钉固定是较稳定的指骨骨折固定方法,能满足早期进行功能锻炼要求。Objective:To investigate the feasibility of using arch-crossing intramedullary nail to fix the phalanx fracture. Methods: The transect fracture were simulated in 12 fresh cadaver phalange specimens. Arch-crossing intrarnedullary nail and ordinary crossing Kirschner's wire were then used to fix the fracture, respectively. The fixture effects were detected using the fatigue test of flexion-extension motion which executed by the MTS to simulate the effects of CPM. Results: Either pre-failure test or post-failure test there were significantly differences between the effects of arch-crossing intramedullary nail and ordinary crossing Kirschner's wire fixture in the horizontal dimension (P〈0.05). However, there was no difference in the sagittal and vertical dimensions. There was no difference of arch-crossing intramedullary nail in the all dimensions except the vertical dimension, while there were significant differences of ordinary crossing Kirschner's wire in all dimensions (P〈0.05). Conclusions: The arch-crossing intramedullary nail fixture is a comparatively stable fixture method which can fulfill the need of the early exercise of the phalange fracture.
