摘 要:目的:为外侧裂手术入路提供显微外科解剖学基础。方法:15例头颅湿标本,模拟手术入路观察外侧裂区解剖结构和相互毗邻关系并进行测量研究。结果:外侧裂前点将外侧裂分割为前后两部分,在93%的标本里外侧裂前点处有外侧裂池存在,该点位于额下回三角部的下方,在Rolandic下点前方(2.1±0.5)cm处,颅骨鳞缝前段下方直径1.5cm的平面上,其毗邻结构组成外侧裂岛盖,该处具有恒定的形态学特征。结论:外侧裂岛盖的特定的结构及外侧裂前点与颅骨、神经的恒定关系使其容易识别,为进一步研究外侧裂处的脑沟脑回提供了可靠的起始标志。Objective: To provide anatomic basis for operative approach in the area of lateral fissure of cerebrum. Methods: This study was based on 15 wet cadaver head specimens, which were explored the features and relationships of lateral fissure of cerebrum. Results: The anterior sylvian point (ASyP) divided this fissure into two portions. For 93% specimens, cistern of lateral sulcus can be found nearing the ASyP. The ASyP was always located inferiorly to the triangular part of the inferior frontal gyrus(IFG), about 2.1 ± 0.5cm in front of the inferior rolandic point(IRP).The ASyP was always located underneath the cranial area of the anterior aspect of squamous suture with the diameter of about 1.5cm.Its adjoining structures that composed the suprasylvian operculum were always permanent in the morphology. Conclusions: The constant structures of the suprasylvian operculum and the consistent relationships between the ASyP and cranium and neural structures can be easily recognized, which provide a reliable landmark for further estimation of the suprasylvian sulcal and gyral structures.
