逆行灌注心脏跳动中主动脉瓣膜替换

摘　　要：用自行设计的经冠状静脉窦逆行灌注装置进行冠状静脉灌注、在心脏有节律的空跳中完成主动脉瓣替换，然后改为非插管顺行灌注，再行二尖瓣替换或其他合并病变的矫治。治疗８例，术后无多巴胺等正性肌力药物依赖和严重心律失常，恢复顺利。这是一种最接近生理状态的心肌保护措施和新的手术方法，具有重要的临床意义和实用价值。Between May 1995 and September 1995,retrograde coronary sinus perfusionwith beating heart was performed in 8 patients undergoing aortic valve replacement.After aorticvalve replacement,the retrograde perfusion was changed to antegrade perfusion in mitral valvereplacement or in correction of other deformities.Dopamine was used postoperatively 0～5μg/ kg.The patients did not develope any serious arrhythmia or low-output syndrome,andall ofthem had a good recovery,Thus,RCSPwith beating heart is a simple,safe and physiolog-ical means of myocardical protection in aortic valve replacement.
