孕妇巨细胞病毒、弓形虫感染及其可能导致宫内的传播

摘　　要：为评价聚合酶链反应（ＰＣＲ）在孕妇巨细胞病毒（ＣＭＶ）及弓形虫（Ｔｏｘ）感染诊断中的应用，发现可能导致ＣＭＶ／Ｔｏｘ母婴传播的因素。采用病毒分离，血清学试验，套式ＰＣＲ加限制内切酶谱分析检测ＣＭＶ。Ｔｏｘ检测采用弓形虫分离，血清学试验及一次性ＰＣＲ。结果发现：（１）１８６例孕妇中，ＣＭＶＤＮＡ、病毒分离、特异性ＩｇＭ及ＩｇＡ阳性率分别为６．５％，３．２％，１．６％及２．７％，ＰＣＲ阳性率高于病毒分离等其它方法（Ｐ＜０．０５）。若以病毒分离及血清学试验为诊断标准，ＰＣＲ法的灵敏度，特异度及正确诊断指数分别达８５．７％，９６．７％及０．８２４。（２）１８６例孕妇中，３例弓形虫ＤＮＡ阳性，阳性率为１．６％（３／１８６）。（３）１０５份母－脐配对血中，６份母血ＣＭＶＤＮＡ阳性，３份配对脐血ＣＭＶＤＮＡ阳性，母－脐传播率为３／６。结果提示：孕妇ＣＭＶ、弓形虫感染的危害较为严重，开展对孕妇ＣＭＶ、弓形虫感染的检测，将有利于优生优育工作的开展。To evaluate the applicability of polymerase chain reaction (PCR)in diagnosis of CMV and Tox infection in pregnant women and to find possible causes which result in the maternal-fetal transmission of CMV/Tox.Diagnostic methods involved were virus isolation,seriological tests,nest-ed PCR and restriction endonucleases analyses to CMV,the etiological isolation of Tox-trophozoites via mice inoculation,seriological tests and single PCR to Tox,Results:(1) Among 186 pregnant women,the positive rate of CMV DNA,virus isolation,nti-CMV-IgM and anti-CMV-IgA were 6.5%,3.2%,1.6%and 2.7%respectively.The positive rate of PCR was significantly higher than that of virus isolation and seriological tests(P<0.05). The sensitivity,specificity and Youden's index of PCR were 85.7%,96.7%and 0.824 respectively when virus isolation and seriological tests were used as a reference method.(2 )Among 186 pregnant women,3 were positive. The positive rate of Tox DNA was 1.6%(3/186).Tox tachvzoites were isolated from the animal inoculation with blood sample being positive in both Tox DNA and CAg.(3)Among 105 maternal umbilical matched pairs,6 maternal blood and 3 matched umbilical blood were positive in CMV DNA,the rate of maternal-fe-tal transimission was 3/6.Conclusion:The morbidity of CMV/Tox infection in pregnant women was serious.The screening program of CMV/Tox infection during pregnancy will be of great significance in eugenics.
