摘  要:目的探讨自身血浆灭活对缓解期乙型慢性活动性肝炎患者血清中乙型肝炎病毒(HBV)的清除作用。方法选择69例缓解期乙型慢性活动性肝炎患者为治疗对象,每2周单采1次血浆,第1次采集血浆400ml,用亚甲蓝和光照灭活病毒,并于6h内冻存。第2次先采集400ml血浆,再回输第1次采集的400ml灭活血浆,同时采集400ml血浆。将第2次采集的800ml血浆病毒灭活后冻存,待第3次采集时回输用。这样第3次可冻存血浆1200ml,第4次可冻存血浆1600ml。共单采7次。最后1次可置换自体灭活血浆2800ml。1个疗程3个月。第1次灭活置换前和最后1次置换后分别留取血清,采用聚合酶链反应结合Taqman荧光探针技术对血清中HBV进行定量测定。结果灭活置换术前、后病毒平均浓度分别为(5.23×105±2.25×102)copy/ml和(2.95×104±1.03×102)copy/ml,灭活置换术前、后HBV含量下降显著(P<0.05)。结论自体血浆病毒灭活置换术对外周血HBV有较强的清除作用。Objective To study the effect of plasma virus inactivation and plasma exchange on clearing hepatitis B virus in serum. Methods 69 patients suffered from chronic hepatitis B were selected for plasma virus inactivation and plasma exchange (PE) ,serum samples were collected for plasma virus inactivation before and after PE,then polymeric chain reaction (PCR) together with fluorescent probe technique was adopted to make quantitative determination of hepatitis B virus in serum samples. Results The mean density of virus before and after PE was (5.23 × 10^5± 2.25 ×10^2) copy/ml and (2.95 × 10^4±1.03×10^2) copy/ml,respectively. Therefore ,the content of virus after PE has decreased significantly (P〈0. 001). Conclusion Plasma virus inactivation and PE has fairly good effect on clearing hepatitis B virus of peripheral blood.
