通腑泻热灌肠剂对急腹症脓毒症患者炎性介质的影响

摘　　要：目的观察通腑泻热灌肠剂对急腹症脓毒症患者血中炎性介质的影响。方法将43例急腹症脓毒症患者随机分为观察组和对照组,分别在常规外科手术及术后处理、抗生素治疗基础上给予通腑泻热灌肠剂和生理盐水灌肠治疗;观察两组治疗前及治疗第3日、第7日血清C-反应蛋白(CRP)、内皮素/一氧化氮(ET/NO)、血栓素B2/6-酮-前列腺素F1α(TXB2/6-Keto-PGF1α)变化。结果观察组治疗第3日和第7日CRP、ET/NO低于对照组,第7日TXB2/6-Keto-PGF1α低于对照组,观察组并发症发生率亦低于对照组。结论通腑泻热灌肠剂可降低急腹症脓毒症患者血中CRP,改善ET/NO和TXB2/6-Keto-PGF1α失调,减轻该症的炎症反应及组织损害,提高临床疗效。Objective:To observe the effect of Tongfuxiere Enema(通 腑 泻 热 灌 肠 剂 ),the Chinese herbs, on serum inflammatory mediators of patients with acute abdom accompanying sepsis.Methods:43 patients were divided into the treatment group and the controlled group on random.On the basis of routine surgical operation,treatment after surgical operation and application antibiotics,patients in the treatment group was treated with Tongfuxiere Enema,and those in the controlled group was treated with saline enema.The changes of serum C- reactive protein(CRP),ET/NO,and thromboxane B2/6- keto- prostaglandin F1α (TXB2/6- keto- PGF1α ) levels of both groups before treatment and on the 3rd day and 7th day after treatment,and clinical therapeutic effect were observed.Results:On the 3rd day and 7th day after treatment,levels of CRP and ET/NO of the treatment group were lower than those of the controlled group. On the 7th day after treatment,the level of TXB2/6- keto- PGF1α of the treatment group was lower than that of the controlled group.The incidence of complications of the treatment group was lower than that of the controlled group.Conclusions:Tongfuxiere Enema can decrease serum CRP of patients with acute abdom accompanying sepsis,correct the disorder of ET/NO and TXB2/6- keto- PGF1α ,reduce the inflammatory reaction and tissue damage,and improve the clinical therapeutic effect.
