肌注重组IFN-γ联合抗结核药治疗肺结核的效果

摘　　要：目的通过观察IFN-γ辅助抗结核治疗对肺结核的疗效,了解其对Thl/Th2动态平衡的干预作用。方法对干预组除常规一线抗结核治疗外,在强化期加用IFN-γ肌注,对照组仅接受常规一线抗结核治疗。观察2组病灶吸收、痰菌阴转情况、血清细胞因子和IgE抗体的变化并作比较。结果干预组与对照组于治疗2、6个月末,在痰菌转阴方面无显著性差异;在病灶吸收方面干预组都明显优于对照组,2组间有显著性差异;治疗2、6个月末干预组IL-4降低和IFN-γ升高都甚于对照组,2组间均有显著性差异。IgE抗体变化与IL-4变化不完全一致。结论外源性IFN-γ对调节Thl/Th2失衡有一定干预效果,可以促使其向Thl优势提前转换并增强。Objective To observe the effect that IFN - γ how to interfere the homeostasis of Th1/Th2 used combine with conventional anti-tuberculosis regimen. Methods IFN - γ was used at initial phase of the treatment （2 months）only in intervention group. Both intervention group and control group were all treated with conventional anti-tuberculosis regimen. The lesion extent in lungs, sputum conversion, serum cytokines and IgE were examined. Results At the end of two months and six months of treatment, there was no significantly difference of sputum conversion rate between the two groups. The lung lesion in intervention group diminished more clearly than in control group. It was statistically significant. The laboratory test showed IL-4 decreased and IFN-r elevated significantly in intervention group than control group. But IgE was not correlated with IL-4 completely. Condusion There was intervention effect of external IFN - γ interfere for the unbalance of Th1 - Th2 during the illness of pulmonary tuberculosis, IFN-r can strengthen and advance Th1 predominance.
